However, according to experts, only about 30% of hypertensive patients achieve their target blood pressure below 130/80 mmHg and a similar rate of diabetic patients control their HbA1C below 7%. This figure shows a big challenge in long-term chronic disease control, requiring more effective and synchronous management solutions.

ESC Highlights 2025 Annual Scientific Conference - Prestigious scientific forum of Vietnamese cardiovascular professionals.
At the annual scientific conference ESC Highlights 2025, organized by Servier Vietnam in collaboration with the Vietnam Cardiovascular Association in Hanoi and Ho Chi Minh City, more than 1,500 experts discussed new updates in cardiovascular treatment and chronic disease management. This year's event focused on a comprehensive approach model called AS ONE, aiming to improve disease control rates for patients with chronic diseases in Vietnam.

Prof. Dr. Nguyen Lan Viet, Vice President of Vietnam Heart Association, shared at the conference.
Speaking at the opening ceremony, Prof. Dr. Nguyen Lan Viet, Permanent Vice President of the Vietnam Cardiology Association, said: " ESC Highlights is an important bridge to help the Vietnamese medical team quickly access global medical advances. Updating knowledge not only improves professional capacity but also contributes to improving the quality of management for patients with cardiovascular diseases in Vietnam. Good management of chronic diseases does not only come from the role of doctors, but also from the cooperation of many departments: from management at all levels, hospital management, to doctors, nurses and patients. All are one - As One - with the common goal of how to improve the rate of disease control for patients ".
AS ONE Model – A solution combining treatment and companionship
Based on the fact that the disease control rate is still low, the AS ONE model was introduced by experts at the workshop as a new approach in chronic disease management, combining advanced treatment solutions and a multi-layered patient support system. The goal of the model is to help patients raise awareness, increase compliance and improve long-term treatment outcomes.
Accordingly, AS ONE includes many elements deployed in parallel: high-quality treatment drugs according to the new ESC 2025 recommendations, encouraging early combination and use of fixed-dose combination pills for optimal control; communication and health education campaigns deployed synchronously, both directly at the hospital and online through the First Day ecosystem, helping to raise public awareness of cardiovascular disease and increase the proactive role of patients; digital tool Elfie supports monitoring, reminding patients to adhere to treatment, and connecting patients and medical teams; along with multidisciplinary cooperation between doctors, pharmacists, nurses, managers and patients themselves - all aiming to improve the rate of control of blood pressure, blood sugar, lipids and heart rate.

Prof. Dr. Dang Van Phuoc, Vice President of Vietnam Heart Association shared at the conference.
Emphasizing the importance of this model, Prof. Dr. Dang Van Phuoc, Vice President of the Vietnam Cardiovascular Association, shared: " We appreciate Servier's support in spreading advanced treatment solutions. The As One solution package is an important step forward in chronic disease management, including not only advanced, optimal treatments but also supportive solutions, helping patients understand the disease and enhance their role in self-management of the disease. This is important in moving towards the goal of comprehensive and sustainable control ."
Accompanying patients for sustainable health
After more than a decade of organization, ESC Highlights has become a prestigious scientific forum for Vietnamese cardiovascular professionals, a bridge to help medical teams update treatment advances from the European Society of Cardiology (ESC). At the same time, the First Day ecosystem, sponsored by the Vietnam Cardiology Association and the Vietnam Endocrine and Diabetes Association, in collaboration with Servier Vietnam since 2016, has also made an important contribution to educating and supporting chronic patients. With more than 100 consulting corners at hospitals, hundreds of patient clubs, more than 20 million visits to the online platform and more than 500,000 downloads of the Elfie application, this project has helped thousands of patients improve their treatment awareness and quality of life.
The AS ONE model is therefore not only a medical initiative, but also a demonstration of the solidarity of the entire system – from doctors, pharmacists, nurses to managers and patients themselves – towards a common goal: improving the rate of chronic disease control, for a healthier and more sustainable community.
