Patient VL (52 years old, Cam Lam commune) was involved in a traffic accident. After the collision, he only experienced mild abdominal pain and therefore did not seek medical attention. When the pain intensified, accompanied by shortness of breath and extreme fatigue, his family took him to Tam Tri Nha Trang General Hospital. Upon admission, the patient was in a critical condition with severe abdominal pain, abdominal distension, pale skin, profuse sweating, difficulty breathing, and unmeasurable blood pressure.

Recognizing this as a case of severe hemorrhagic shock due to blunt abdominal trauma, the hospital immediately activated the "red alert" protocol. The emergency team performed bedside ultrasound and urgent paraclinical procedures. The results showed a large amount of fluid in the abdominal cavity, indicating severe internal bleeding. The patient was diagnosed with a ruptured spleen due to blunt abdominal trauma and was immediately transferred to the emergency operating room.

During the surgery, the doctors observed a complex splenic rupture in the lower pole with a star-shaped tear approximately 8-10 cm long, causing massive bleeding in the abdominal cavity. The team decided to remove the entire spleen to control the bleeding, and simultaneously transfused 5 units of blood and 6 units of fresh plasma to compensate for the lost blood volume.

After the surgery, the patient was transferred to the Intensive Care Unit for special monitoring. Thanks to intensive treatment, after 4 days, the patient was alert, responsive, vital signs were stable, and the patient was able to eat, drink, and converse with family members. The patient is expected to be discharged in the next few days.

Dr. Vo Phuoc Toan emphasized that this is a typical emergency case demonstrating the importance of the "golden hour" in managing blunt abdominal trauma. A ruptured spleen is an extremely urgent surgical emergency that can be life-threatening within hours if not diagnosed and treated promptly. The success of this case was due to the hospital's prompt implementation of the emergency protocol and the multidisciplinary collaboration during surgery.

The ruptured spleen was completely removed to save the patient's life.

Doctors advise that after any injury, especially traffic accidents, if symptoms such as progressively worsening abdominal pain, bloating, pale skin, sweating, dizziness, and shortness of breath appear, people should immediately go to the nearest medical facility for examination and timely treatment. They should not be complacent and self-monitor at home, as this may cause them to miss the "golden time" for emergency treatment of injuries.

The patient's health is stable after surgery.
