The 30-year-old patient was admitted to the emergency room in critical condition with a cavernous hemangioma hemorrhage, while two cerebral aneurysms remained immobile in extremely dangerous locations.

Dr. Nguyen Van Tuan, Head of the Neurosurgery Department at People's Hospital 115 (Ho Chi Minh City), said that they have successfully treated a complex and rare case of cerebral vascular malformation.

Earlier, Mr. NNHV (30 years old) was admitted to the emergency room with severe headaches, dizziness, continuous vomiting, and unsteadiness. Because the patient was so young and had no significant underlying medical conditions, these acute symptoms caused his family great concern.

The results of the magnetic resonance imaging (MRI) scan of the brain's blood vessels surprised even the most experienced doctors. Mr. V. not only had one lesion but also three "time bombs" waiting to explode in his head.

Specifically, the imaging system revealed a cavernous vascular tumor (cavernoma) in the left cerebellum that had become complicated, causing cerebral hemorrhage. Even more alarming, the screening revealed two additional cerebral aneurysms, one of which was located directly in the vertebral artery, a location with an extremely high risk of rupture.

"Cerebral hemorrhage caused by vascular tumors often progresses silently and is difficult to detect early in young people because many cases have no clear symptoms. When acute bleeding occurs, the risk of death or permanent disability is very high," said Dr. Tuan.

Dr. Tuan noted that cavernous hemangioma is a rare congenital vascular malformation. In Mr. V.'s case, after more than 20 days of intensive medical treatment without improvement, the doctors were forced to intervene surgically to stop the risk of re-bleeding.

However, the pressure of the tumor removal surgery could rupture the two accompanying aneurysms. If the aneurysms rupture on the operating table, the patient will experience massive bleeding and certain death.

To save the 30-year-old man's life, the doctors decided to implement a step-by-step "mine-clearing" strategy. First, they treated the two aneurysms endovascularly, and only then proceeded with microsurgery to remove the tumor.

Dr. Tran Thanh Vu, Head of the Neurovascular Intervention Unit, said the team applied the advanced Balloon Test Occlusion (BTO) technique to assess whether the contralateral cerebral circulation was capable of compensating after occluding the blood vessel containing the dangerous aneurysm.

After the aneurysm "mine" was successfully neutralized, the patient recovered well and was well-prepared for the decisive battle.

Immediately afterward, a microsurgery lasting over 3 hours was urgently performed. With skillful hands and the support of modern equipment, the medical team completely dissected and removed the cavernous vascular malformation and hematoma in Mr. V.'s cerebellum without damaging the surrounding healthy brain tissue.

Just two days after the surgery, Mr. V. was fully conscious, and his previous symptoms of vomiting, dizziness, and headache had disappeared. Currently, the young man is undergoing physical therapy to help him return to his normal life and work as soon as possible.

Dr. Tuan warns that stroke or cerebral vascular malformations are no longer diseases of the elderly. To prevent these diseases, people need to control their blood pressure well and avoid prolonged stress. When the body shows abnormal neurological signs such as dull headaches, numbness or weakness in the limbs, dizziness, loss of balance, etc., it is necessary to go immediately to specialized medical facilities for screening to avoid unfortunate consequences.
