Lak District: Primary health care facilities lack human and material resources

Lak district's health sector is still facing many difficulties in performing professional and technical tasks due to lack of both human and material resources.

Doctor Le Manh Hung, Deputy Director of Lak District Medical Center, said that the Center currently has 3 functional departments, 7 specialized departments, manages 11 commune and town medical stations with more than 240 staff, employees, and doctors.

Lak District Medical Center is currently assigned a target of 120 hospital beds, however, on average, the Center receives about 170 - 200 patients per day, of which only 50 - 70 patients stay per day, the hospital bed usage capacity only reaches about 50% of the assigned target.

The reason for the above situation of hospital bed vacancy is because a part of the population with stable economic conditions receive medical examination and treatment according to high-quality service packages at private, provincial and central hospitals; the remaining population, mostly ethnic minorities, with difficult economic conditions, do not pay due attention to health care, only when there are clear signs of illness or the illness becomes severe, are they hospitalized for treatment, or treated in an emergency situation and must be transferred to a higher level.

In addition, Lak District Medical Center has not yet been able to perform many specialized and high-tech techniques. Explaining this issue, Dr. Le Manh Hung said that the equipment and machinery to perform specialized techniques have been registered for centralized procurement bidding in recent years but have not been implemented. This is also a common situation in the health sector when the legal regulations on bidding for drugs, chemicals, testing supplies, and medical equipment issued in recent times have many new points, and the units have not yet fully grasped them to apply to procurement.

In 2024, Lak District Medical Center implemented 10 bidding packages, including 3 national-level drug bidding packages, 1 local-level drug bidding package, and 6 grassroots-level bidding packages. Although there has been initiative in providing drugs for patients, there is still a local shortage, especially in the bidding transition period between two consecutive years. For example, the community mental health protection program in Lak district. The total number of mental patients treated at the Center is more than 400, but the number of patients using drugs is only nearly 50%. The reason is that the drug supply is still lacking and not suitable for the disease, so the number of patients coming to the facilities for treatment has decreased, leading to a decrease in the number of patients using drugs according to the treatment regimen, people mainly receive drugs at the Provincial Mental Hospital.

In addition to the above difficulties, the human resources for medical examination and treatment have basically met the demand in terms of quantity but the quality is not deep. Doctor Le Manh Hung gave an example that currently a general practitioner can be flexibly transferred from one department to another to support professional expertise, but a specialist cannot be transferred. For example, a surgeon cannot be transferred to the internal medicine department, and surgery requires an anesthesiologist. Recently, Lak District Medical Center has been supplemented with a specialist I in anesthesia, however, in the process of performing the task, it is necessary to synchronize the accompanying team in terms of expertise, so the Center has sent doctors to continue studying and training.

On the other hand, the "brain drain" from the public to the private sector in the health sector is also the reason why the Center's already limited human resources are even more deficient. From 2019 to now, Lak District Medical Center has had 8 employees quit or transferred jobs, including 3 doctors. The main reason is that after the COVID-19 pandemic, the work pressure on medical staff and doctors is too great, they have to face many difficulties and dangers but the support regime is not timely and not appropriate. The lack of human resources leads to difficulties in arranging people on duty, coordinating emergency care for patients...
