After being admitted to a local medical facility, she was diagnosed with a suspected soft tissue abscess due to a bacterial infection. Despite treatment with antibiotics, her condition did not improve, her blood pressure dropped and she had to be maintained on vasopressors to maintain her blood pressure. Shortly thereafter, the patient was transferred to the Emergency Department, Central Hospital for Tropical Diseases.
At the hospital, patient X's condition continued to worsen. Doctors noted that the entire right shoulder and arm were red and swollen, with increasing pain. Ultrasound and CT scans showed diffuse inflammation under the skin of the arm, accompanied by synovitis of the shoulder joint.
After ruling out other sources of infection, doctors confirmed that the joint injection at a private clinic was the sole cause of septic shock. Tests also showed severe systemic inflammation and high white blood cell counts.
After several days of intensive treatment, the patient's blood pressure stabilized and inflammation indicators decreased significantly, and she was out of danger.

The needle is a direct route of entry, and if not sterile, bacteria can enter soft tissue, causing infection, even sepsis and death if not treated promptly.
This is not the only case. Recently, the Department of Geriatrics - Musculoskeletal System of Bai Chay Hospital has also received many patients with severe complications after joint injections at private clinics.
A typical case is patient BTM (24 years old, Quang Ninh ) after receiving a joint injection at a private clinic due to wrist pain, got an infection in both the wrist and right shoulder joints, with symptoms of swelling, heat, redness and limited mobility. Another case, patient NTK (54 years old, Quang Ninh) got complications of joint infection and lower limb venous thrombosis after receiving a knee injection at a private clinic.
According to information from the Vietnam - Sweden Uong Bi Hospital, the Neuropsychiatric - Musculoskeletal Department has recently received many patients with knee joint infections after injections at facilities that do not ensure sterility. These cases not only require medical treatment but also surgery to handle serious complications.
Joint injections should only be used when conventional treatments have failed. The procedure requires a strictly sterile environment and must be performed by a qualified specialist.
If sterile conditions are not ensured, patients may face dangerous complications such as joint infection, bone necrosis, tendon perforation, even sepsis, endocarditis, which are life-threatening.
Dr. Nguyen Tuong Van, Head of the Department of Geriatrics - Musculoskeletal, Bai Chay Hospital, said that joint injections should only be performed in cases of soft tissue diseases near the joint such as tendonitis, bursitis, rheumatoid arthritis or early stage osteoarthritis.
However, there are also contraindications to joint injection such as septic arthritis, systemic infection, patients with blood disorders or those who have had artificial joint replacement.
Doctors also emphasized that joint injections at facilities that do not ensure quality, especially those that are not licensed or do not ensure sterile conditions, can cause dangerous complications. To avoid the risk of infection and serious complications, people should follow the instructions of specialists and absolutely do not arbitrarily inject joints at facilities that do not ensure quality.
