Hives occur when the body reacts to allergens. When exposed to allergens, histamine and mediators are released, causing blood vessels to dilate and fluid to escape, accumulating under the skin, causing acute or chronic edema in the dermis, making the skin red, swollen and itchy.

1. Common causes of hives

Urticaria is a reaction of the capillaries under the skin and mucous membranes to allergens from inside or outside the body, causing local swelling, making the skin swell with symptoms of itching and discomfort. Urticaria can appear in one area of ​​skin or many different areas.

Hives are divided into two types: acute hives (lasting no more than 6 weeks) and chronic hives (lasting more than 6 weeks), which can last for months or years.

The disease can occur in anyone but is more common in women aged 40-60. In most cases (80-90%), no specific cause can be found. The cause of urticaria is complex; in the same patient, there may be multiple factors causing the disease.

- Due to food: Many foods can cause hives such as milk, eggs, sea fish, shrimp, crab, shellfish, cheese, canned food, fish sauce, soy sauce, fermented bean curd, chocolate, fermented drinks (wine, beer), tomatoes, watercress, cucumbers, potatoes... Even "healthy" foods can cause hives.

- Due to drugs: All types of drugs and routes of administration can cause hives. The most common are beta-lactams, followed by cyclin, macrolides, chloramphenicol. Non-steroidal anti-inflammatory drugs, vitamins, vaccines, serums, antimalarial drugs, ACE inhibitors... can all cause the disease. Even glucocorticoids, prednisolone, dexamethasone and antihistamines such as clarithromycin, theralen... can also cause hives.

- Due to respiratory agents: Straw, pollen, house dust, warehouse dust, feathers, cigarette smoke, mold.

- Due to infection: Viral infection (hepatitis B, C), bacterial infection in the ear - nose - throat, digestive tract, teeth, urinary - genital; intestinal parasite infection or candida infection in the skin and internal organs.

- Due to contact with organic and chemical substances: Cosmetics, lipstick, perfume, hair dye, nail polish, soap, food colorants and preservatives.

  • Physical urticaria (accounts for more than 50% of chronic urticaria):

- Dermatographism.

- Hives caused by movement, emotions such as fatigue, exertion, stress.

- Hives caused by cold, heat, sunlight, or water.

- Urticaria due to other diseases: Lupus erythematosus, vasculitis, diabetes, hyperthyroidism, cancer.

In addition, urticaria due to family factors accounts for 50-60%. If only one parent has urticaria, about 25% of children will have the disease; if both have it, the rate is up to 50%. Some cases of spontaneous (idiopathic) urticaria have no known cause, accounting for about 50%.

2. Common symptoms of hives

Urticaria lesions are red, edematous papules of variable size, slightly raised on the skin, red or paler than the surrounding skin. The papules appear and disappear quickly, can be localized or spread throughout the body.

In loose tissue areas such as the eyelids, lips, external genitals, etc., the edema causes swelling - called angioedema (Quincke's edema). If Quincke's edema occurs in the larynx or digestive tract, it can cause severe difficulty breathing, abdominal pain, diarrhea, low blood pressure, cardiovascular disorders or anaphylactic shock.

In most cases, the more you scratch, the more it itches. Some people only feel a stinging or burning sensation. The rash usually disappears after a few minutes to a few hours, leaving no trace, but it can easily recur in waves.

Therefore, if hives persist, recur continuously or are accompanied by difficulty breathing, swelling of the lips and eyes, the patient needs to go to a medical facility for timely treatment.

Summary: Urticaria is a skin condition characterized by red, swollen, and intensely itchy rashes that can be transient or last for weeks. The disease often flares up when exposed to irritants such as cold weather, allergic foods, insect bites, stress, or environmental pollution.

Avoid known triggers, wear warm clothes when it is cold, avoid bathing in water that is too hot, keep the skin and house clean, wash bedding regularly and bathe after going outside. Do not scratch or rub vigorously to avoid spreading the injury; you can apply a cool compress to reduce itching.
