Repairing cesarean section scars is an effective treatment for chronic menorrhagia, uterine fluid accumulation, and secondary infertility caused by damage to the previous surgical incision.

On August 5th, Nghe An Obstetrics and Pediatrics Hospital announced that doctors in the Gynecology and Obstetrics Department had successfully performed laparoscopic surgery on a patient who had suffered from secondary infertility for many years due to a cesarean section scar defect, causing fluid accumulation in the uterine cavity.

The patient, Ms. LTMM (34 years old, residing in Nghe An), was admitted to the hospital with persistent dull abdominal pain and dark vaginal discharge during her menstrual period.

According to the patient's medical history, she had a cesarean section 13 years ago, currently has one child, and has undergone infertility treatment for many years without success. Previously, she had undergone embryo transfer (IVF) but it was unsuccessful.

Upon examination, doctors determined the cause was fluid accumulation at the site of the old surgical scar on the uterus, affecting the embryo's ability to implant.

Notably, the site of the previous surgical scar caused continuous fluid accumulation in the uterine lining, leading to prolonged vaginal bleeding, with dark-colored vaginal discharge almost throughout the cycle. The patient and her family decided to choose the Obstetrics and Gynecology Department of Nghe An Obstetrics and Pediatrics Hospital for the intervention.

During the surgery, the doctors noted that the entire anterior surface of the uterus was adhered tightly to the abdominal wall. Using meticulous and careful laparoscopic techniques, the surgical team successfully dissected and exposed the uterine isthmus, removed the old surgical scar, and repaired it.

The surgery was safe, with no blood loss and no need for a conversion to open surgery. Post-surgery ultrasound results showed no remaining scarring or fluid accumulation in the uterine cavity. The patient's symptoms of menorrhagia and dark vaginal bleeding also ceased almost immediately.

According to MSc. Dr. Nguyen Huu Hoai, Deputy Head of the Obstetrics and Gynecology Department, laparoscopic surgery to repair cesarean section scars is an effective treatment method for cases of chronic menorrhagia, uterine fluid accumulation, and secondary infertility due to mechanical damage at the old surgical site.

The rate of improvement in symptoms of menorrhagia and metrorrhagia ranges from 65% to 82.4%, while also improving the chances of natural conception or assisted reproduction through IUI and IVF.

Over the past period, the Obstetrics and Gynecology Department of Nghe An Obstetrics and Pediatrics Hospital has successfully implemented this technique for many patients, bringing significant results in the treatment of secondary infertility. The method not only helps to improve pregnancy rates but also shortens recovery time and reduces costs for patients.

Doctors recommend that, to detect early potential gynecological lesions affecting fertility, women should have regular check-ups at specialized facilities, especially when experiencing unusual symptoms in their menstrual cycle.
