Autonomy to serve better, not to collect more

As the head of a tertiary hospital in the health system, Associate Professor, Dr. Dao Xuan Co, Director of Bach Mai Hospital, shared solutions to implement the goals of public health care stated in the draft documents submitted to the 14th National Party Congress.

According to Associate Professor, Dr. Dao Xuan Co, the draft documents submitted to the 14th Congress set the target of Vietnam's Human Development Index (HDI) reaching 0.78 by 2030; average life expectancy at birth at 75.5 years; minimum healthy life expectancy at 68 years... These are very humane goals, clearly demonstrating our Party's vision of people as both the center and the driving force of development. However, to achieve these targets, it is necessary to synchronously deploy 4 groups of solutions.

First, continue to invest in preventive medicine and primary health care, considering this a key factor to help people "be healthy right from the community".

Second , promote prevention and control of non-communicable diseases, care for the health of the elderly, improve nutrition, physical fitness, and a healthy living environment.

Third, improve the quality of primary health care services, ensuring that all people, even those in remote areas, have access to quality medical examination and treatment services.

Fourth, promote digital transformation, apply science and technology in healthcare, towards a value-based healthcare model, in which people's health outcomes are the most important measure.

The health sector, including central hospitals such as Bach Mai, will continue to play a leading professional role, supporting the improvement of capacity at lower levels, contributing to the successful implementation of the goal of "health for all, leaving no one behind".

Notably, the draft documents submitted to the 14th Party Congress and Resolution No. 72-NQ/TW also emphasize the organization of periodic health check-ups for the entire population and strengthening health care for the elderly.

For this policy to be feasible, it is necessary to perfect the universal health insurance policy, ensuring that all people receive basic periodic health check-ups, with the cost partly covered by the health insurance fund. Each person needs to have an electronic health record, updated regularly to continuously manage their health status.

Along with that is the strong development of the grassroots health care network and the family doctor model, closely linking prevention-treatment-rehabilitation, especially in caring for the elderly in the community.

This is a sustainable approach to help reduce the burden on the upper level, while providing regular, continuous and closer health care to the people. In addition, it also promotes communication, health education , and raises people's awareness of the benefits of regular check-ups and early detection of diseases.

As a special-class and final-level hospital, Bach Mai Hospital will continue to accompany the Ministry of Health in training, transferring techniques and providing professional support to lower-level hospitals, contributing to the concretization of this policy, towards the goal of "health for everyone, of all ages".

To do a good job of taking care of people's health, the human resources must not only be sufficient in quantity but also of quality. The draft documents submitted to the 14th Party Congress clearly stated: "Develop, promote and reward intellectuals and high-quality human resources in key areas, including health care". This is a strategic orientation, demonstrating the deep concern of the Party and State for the medical staff.

However, according to the Director of Bach Mai Hospital, in addition to focusing on new training, retaining good doctors in the public sector is also quite important, it is necessary to synchronously implement three groups of fundamental solutions: improve salary regime, income and working conditions, so that doctors can make a living from their profession, feel secure in their dedication; build a healthy academic, research and professional development environment, create opportunities for doctors to study, be creative, and affirm their professional values; innovate the hospital autonomy mechanism associated with social responsibility, allowing units to be more proactive in using legal resources to take care of the lives of staff, but still put serving the people first.

Regarding the policy of public hospital autonomy, this is a major, correct and inevitable policy to promote initiative, creativity, improve service quality, and reduce the burden on the State budget. However, autonomy does not mean loosening management or commercializing public health care. There needs to be mechanisms to help hospitals implement autonomy, but the goal is still "Developing a fair and effective health care system, taking serving the people as the highest goal".

Therefore, it is necessary to perfect the mechanism and policies on finance, service prices and bidding, creating a clear and transparent legal corridor so that hospitals can be proactive in managing and using legal resources.

The State needs to continue to ensure its leading role in providing public health services, especially for the poor, people in disadvantaged areas and non-profitable health sectors.

Promote publicity, transparency and social supervision in autonomous activities, so that all resources are directed towards improving the quality of people's health care.
