Mr. Giàng A Phình (32 years old, residing in Mường Toong commune, Điện Biên province) was transferred to Điện Biên Provincial General Hospital in critical condition, with his left hand severely crushed after being caught in the shaft of a cassava shredding machine on the afternoon of November 24th.

Immediately after the accident, local residents helped by applying pressure bandages to stop the bleeding and taking the victim to Muong Nha Medical Center for initial first aid.

Here, patients receive pain relief, antibiotics, SAT injections, and temporary compression bandaging before being transferred to the provincial level.

Upon admission, doctors noted a jagged injury to the back of the left hand, with severe crushing of the second and third fingers, exposing the bone, along with proximal phalanges and dislocations of the second and third phalanges. The patient was diagnosed with complex crush injury to the left hand and was immediately taken to the operating room.

Dr. Nguyen Duc The, Deputy Head of the Department of Trauma, Orthopedics and Burns at Dien Bien Provincial General Hospital, said that the injury to the second finger was too severe, the structure was completely destroyed and there was no possibility of preservation.

To prevent necrosis and infection from spreading to the rest of the hand, the medical team had to amputate the second finger.

During the surgery, the doctors performed bone fixation, tendon repair, and skin flap transfer to cover the soft tissue in order to preserve as much function as possible of the remaining hand. Thanks to timely referral and surgical intervention, the patient is out of danger and is being monitored post-operatively in the ward.

Doctors warn that accidents involving cassava shredders, grinders, and other mechanical equipment used in production always pose a high risk of causing crushing injuries to limbs.

People must strictly adhere to safety rules when operating machinery, including inspecting the machine before use, wearing appropriate protective equipment, avoiding loose or cumbersome clothing, not putting hands into areas where the machine is operating, and completely disconnecting the power supply when dealing with a malfunction.

In case of injury, priority should be given to properly controlling bleeding and transporting the victim to the nearest medical facility as soon as possible. Self-treatment at home should be avoided as it can easily increase the risk of infection and serious complications.
