At the same time, the producer of Detective Kien: Headless Case also revealed who plays the ghost in the film. According to the crew, this special role is played by Tran Ngoc Tu - an athlete of the Ho Chi Minh City Vovinam team. He impresses with his height of 2.2m, weight of nearly 120kg, and has won many outstanding achievements, including the gold medal at the 2019 National Vovinam Championship.

Director Victor Vu affirmed his desire to design the ghost character of Detective Kien: Headless Mystery based on the image of people who died underwater, with their bodies swollen. Therefore, the ghost image in this film is quite different from previous films. Dinh Ngoc Diep recounted: "Every day, the person playing the ghost role has to spend 6 hours on makeup. It can cost hundreds of millions of dong each time you appear. Thanks to that, the horror scenes also create real effects. Yet Victor Vu didn't even use all of it. Sometimes there are only the legs, the head or just the hands."

To complete this look, the makeup team used special silicon to create the head and body, at a cost of up to 300 - 600 million VND. The character of athlete Tran Ngoc Tu was filmed for 4 days in the mountains, where the climate is harsh and the terrain is difficult. However, due to the film's pacing and the moderate level of horror, many scenes with this character were cut by director Victor Vu in the final version.
