Expired vaccines given to 4 infants

According to information from the Ministry of Health , on May 11th, the Department of Preventive Medicine received Report No. 599/KSBT-PCBTN from the Thanh Hoa Provincial Center for Disease Control dated May 10th, 2023, regarding a vaccination incident at the Thang Binh Health Station, Nong Cong District.

According to the Center's report, on the morning of May 9th, the Health Station administered the first dose of the Hexaxim vaccine, which expired in March 2023, to 4 out of 6 children aged 2 to 4 months.

Four newborns received expired vaccinations; the Ministry of Health has requested an investigation. (Figure 1)

In accordance with the regulations on vaccine management and use stipulated in Government Decree No. 104/2016/ND-CP dated July 1, 2016, on vaccination activities and the regulations of the Ministry of Health on vaccination, the Department of Preventive Medicine requests the Department of Health to organize an investigation and convene a professional advisory council to assess the causes of severe adverse events during vaccine use, evaluate and conclude on the above-mentioned case, implement corrective actions, and report as required;

Review and clarify the responsibilities of units and individuals who violated regulations, and seriously rectify the implementation of vaccination procedures at the local level, preventing errors from occurring during the vaccination process.

Review the entire vaccination process to rectify vaccination activities in the area; Organize retraining for staff involved in vaccination work in the area if necessary to ensure safe vaccination practices in accordance with regulations.

Instruct local media outlets to provide comprehensive communication to avoid causing panic among the public.

The National Institute of Hygiene and Epidemiology provides technical support and guidance to localities in investigating and verifying the causes of post-vaccination incidents and proposes remedial actions for the future.

The report should be submitted to the Department of Preventive Medicine before May 13, 2023, for compilation.
