Following the release of the June Patch Tuesday update (version KB5094126), Microsoft officially confirmed an unusual issue related to the 'Recycle Bin' tool. This error appeared shortly after a series of user reports of other serious problems such as loss of OneDrive and Dropbox access, BitLocker recovery lockout, and the Blue Screen of Death (BSOD).

In terms of appearance, when a user attempts to permanently delete an item from the Recycle Bin, the confirmation dialog will display the system's internal filename code instead of the actual name. For example, a regular file named abc.png will display a strange character code like $Rxxxxx.png when deleted. However, the effect is limited to the dialog box display; the original filename remains correct in the overall list, and when restored, the file returns to its original folder with the correct name.
This display issue is widespread, affecting all currently supported versions of Windows, including both personal computers and servers. In the client segment, the error appears in Windows 10 version 22H2, Enterprise LTSC versions, and Windows 11 generations such as 23H2, 24H2, 25H2, and 26H1. In the server segment, all platforms from Windows Server 2012, 2016, 2019, 2022 to the latest version, Windows Server 2025, have reported similar anomalies.
Currently, Microsoft is working to develop a permanent solution and is committed to releasing it in a future Windows update, although it's unclear whether they will wait for the next Patch Tuesday update or release an emergency patch. Commercial and enterprise customers can implement a temporary workaround immediately to stabilize their systems. However, these organizations are advised to proactively contact Microsoft's enterprise support directly for further detailed instructions.
