Approximately three weeks before hospitalization, a two-month-old girl of the Hmong ethnic group from Van Chan, Yen Bai, developed scattered red papules on her buttocks. These papules then progressed into clusters, patches, and blisters, spreading to both legs, arms, and neck.

The family took the child to a private clinic, where he was diagnosed with dermatitis, given ointment, and bathed in green tea water for two days, but his condition did not improve.

The illness continued to worsen, the blisters ruptured, releasing a yellowish fluid mixed with blood. In the following days, the child developed a high fever of 38°C, became restless and crying, so the family took the child to a medical facility for examination. The examination and tests there revealed that the child had congenital syphilis accompanied by severe anemia.

The child was transferred to the National Hospital for Tropical Diseases in a state of malnutrition, severe anemia, high fever, and blistering skin lesions on the arms, legs, and torso.

The child received emergency antibiotic treatment for two weeks according to the Ministry of Health 's protocol, combined with blood transfusions to improve anemia and intensive care and nutrition.

After 5 days of treatment, the child's skin lesions gradually subsided, the fever subsided, and the child's appetite improved. After 2 weeks of intensive treatment, the child was discharged in stable condition, with positive weight gain.

According to Dr. Nguyen Manh Truong, from the Pediatrics Department of the Central Hospital for Tropical Diseases, the full-term baby, weighing only 1.6 kg, a clear sign of fetal malnutrition, may have been affected by congenital syphilis. "Notably, both parents were diagnosed with syphilis but were unaware of it beforehand," Dr. Truong stated.

Congenital syphilis not only causes malnutrition and anemia in children, but can also damage vital organs such as the liver, heart, eyes, ears, and even progress to the nervous system, causing serious complications and sequelae.

This disease is completely preventable if pregnant women proactively undergo syphilis screening early. Early detection and treatment not only protect the mother's health but are also crucial for ensuring the child's healthy development. Therefore, regular prenatal checkups and adherence to medical instructions are the best ways to prevent congenital syphilis.
