Patient saved after suffering from rare 'urination through feces' condition.

On April 24th, Quang Tri Provincial General Hospital announced the successful surgery of a rare bladder-sigmoid colon fistula, saving the patient from the risk of prolonged infection.

A 74-year-old male patient residing in Nam Cua Viet commune, Quang Tri province, was admitted to the hospital with abnormal symptoms such as passing stool (yellowish, foul-smelling fluid in the urine), gas in the urine, and painful urination. After clinical examination and paraclinical tests including abdominal ultrasound, CT scan, colonoscopy, and cystoscopy, the general surgery department determined that the patient had a bladder-sigmoid colon fistula, while ruling out cancer as the cause.

According to doctors, this is a rare condition that creates an abnormal connection between the bladder and the sigmoid colon, causing stool to leak into the bladder and lead to severe infection.

If left untreated, the disease can lead to prolonged urinary tract infections, chronic cystitis, pyelonephritis, and even sepsis.

Given the patient's multiple comorbidities, including hypertension, gout, and pulmonary tuberculosis, the surgical team decided to perform laparoscopic surgery. During the operation, the sigmoid colon and upper rectum were found to be tightly adhered to the posterior surface of the bladder, with thick, inflamed fibrous tissue, making dissection difficult. After treatment, the doctors discovered a 0.5 cm fistula connecting the colon to the bladder.

The surgical team excised the fistula down to healthy tissue, sutured the bladder closed, and simultaneously resected the diseased sigmoid colon and restored digestive continuity using sutures. After 6 days, the patient recovered well, was able to walk and eat normally, no longer experienced painful urination, and had clear urine.

Hospital representatives stated that the surgery was performed safely, thoroughly addressing the injury using minimally invasive techniques. This result further affirms the hospital's professional capacity in implementing advanced surgical techniques, meeting local treatment needs.
