
The Bac Tan Uyen commune health station has established 6 continuous health care teams linked to residential areas , with the participation of doctors, nurses, pharmacists, public health workers, and community health collaborators.
Launched in April 2026 as part of the City 's health sector's initiative to innovate primary healthcare activities, the Bac Tan Uyen Commune Health Station has established 6 continuous health care teams linked to residential areas , with the participation of doctors, nurses, pharmacists, public health workers, and community health collaborators. These teams are assigned to oversee all 18 hamlets in the commune, directly visiting each household to conduct health screenings, create health records, provide disease prevention advice, and monitor health regularly.
After two months of implementation, the teams completed the registration and health management for 429 people aged 75 and older, exceeding the initial local statistics. Notably, through home health check-ups, medical staff discovered 94 new cases of illness or chronic diseases that were not under stable control, enabling timely referral for management and treatment at the commune health station .
The survey results also showed that common diseases among the elderly in the area include hypertension, diabetes, dyslipidemia, musculoskeletal disorders, and gastrointestinal diseases. Early detection, continuous monitoring, and timely intervention help limit severe complications, reduce unnecessary hospitalizations, and improve the quality of life for the people.
In addition to conducting health check-ups and managing health, the teams also organize consultations, health education and communication, guide people to change behaviors harmful to their health, and connect with Binh Duong General Hospital through online support groups to improve the effectiveness of patient care in the community.
According to the assessment of the Bac Tan Uyen Commune Health Station , the model has brought many practical benefits. Elderly people, especially those with mobility difficulties, can access healthcare services right at home. Chronic disease management has become more proactive and continuous. The relationship between healthcare workers and the people has been strengthened, contributing to building community trust in primary healthcare.
Initial results in Bac Tan Uyen show that the model of continuous health care teams linked to residential areas not only helps improve the effectiveness of health stations but also realizes the goal of shifting the focus from treatment to disease prevention, placing people at the center of the health system in accordance with Resolution No. 72-NQ/TW. This is also one of the important directions for developing grassroots healthcare effectively, building a continuous, comprehensive, and people-friendly health care network in the new phase.
In the coming period, Bac Tan Uyen commune will continue to expand the scope of care to include people aged 60 and above, while completing community health data, aiming for the goal of ensuring that all citizens receive regular health management, monitoring, and care right in their place of residence.
