Pregnant woman in cardiac arrest due to uterine rupture

Ho Chi Minh City, a 39-year-old pregnant woman, pregnant for the third time, suddenly had severe abdominal pain and then went into cardiac arrest and stopped breathing. Doctors at Tu Du Hospital saved both mother and child.

The pregnant woman, living in Binh Duong , had given birth naturally to a 12-year-old and an 8-year-old. Two years ago, she had surgery to remove uterine fibroids and then had a contraceptive implant inserted. After that, she had irregular menstruation and metrorrhagia, so the implant was removed.

When she was 25 weeks pregnant, the doctor discovered placenta accreta, the most severe form, where the placenta penetrates the uterine peritoneum and can invade nearby organs.

On the morning of June 23, at nearly 34 weeks pregnant, she suddenly had severe abdominal pain to the point of not being able to breathe. On the way to Tu Du Hospital for emergency treatment, the patient fainted, and by the time she arrived at the emergency department, she was in a coma, her heart and breathing had stopped, her pulse and blood pressure were unmeasurable, and her abdomen was so distended that it was difficult to determine the fetus.

The doctor determined that the patient had a ruptured uterus, hemorrhagic shock, and was in critical condition, and activated the red alert procedure to mobilize medical support. The patient was resuscitated, given external cardiac compression, adrenaline injection, intubated, an intravenous line established, and an emergency cesarean section to deliver the baby.

The baby boy was born prematurely, unresponsive, cyanotic, and was intubated, given a balloon pump, and taken to the neonatal department for mechanical ventilation. The mother had her uterus removed, was transfused with more than 3.3 liters of blood and blood products, and her heart started beating again.

Normally, the patient is in a coma, has cardiac arrest, massive blood loss, and is at risk of irreversible brain damage, multiple organ failure, blood clotting disorders, lung damage, and infection. Fortunately, this patient "recovered beyond imagination," according to the doctors. Three days after the life-threatening surgery, she was able to walk on her own, and ultrasound and blood tests showed that her health was stable.
