Does drinking coffee increase uric acid levels?

SKĐS - Coffee is a popular beverage worldwide, but should people at risk of high uric acid levels or gout drink coffee?

Uric acid is a substance produced when the body breaks down purines—a naturally occurring compound found in many foods and beverages. When uric acid levels become excessively high in the blood, they easily crystallize into urate salts in the joints and tissues, causing an acute inflammatory condition called gout.

1. Results from scientific studies have not shown that coffee increases uric acid levels.

Scientists analyzed data from several large studies examining the link between coffee consumption, blood uric acid levels, and gout risk. The results showed that coffee drinkers generally did not tend to have elevated uric acid levels—in fact, uric acid levels may have been lower in the coffee-drinking group compared to the non-coffee group.

This study used data from nearly 15,000 adults to assess uric acid levels and coffee consumption and found that those who drank coffee tended to have lower blood uric acid levels. This is a noteworthy finding, as gout is a clinical consequence of high uric acid levels. This is not just a finding in a few studies, but is seen in the majority of data collected on coffee and gout.

This suggests that coffee may exert its effects through other biological pathways such as anti-inflammation and increased uric acid excretion through the kidneys, rather than simply reducing uric acid levels.

2. What should people at high risk of elevated uric acid or gout do?

1. Drink coffee in moderation: Studies have not shown that drinking coffee increases the risk of high uric acid or gout. On the contrary, many studies have found that coffee drinkers have a lower risk of gout. Therefore, if you are healthy, drinking coffee in moderation (1-3 cups/day) is generally considered safe and there is no need to avoid it for fear of increased uric acid.

2. Pay attention to the ingredients of your drink: Drinking coffee with lots of sugar, syrup, and cream can negatively impact your overall health, especially if you have high insulin levels or are obese—factors associated with increased uric acid and gout. Therefore, avoid overly sweet pre-made drinks and opt for black coffee or coffee with less sugar instead.

3. Monitor individual symptoms: While overall evidence doesn't show coffee as the "culprit" behind increased uric acid levels, and each body reacts differently, if you experience worsening joint pain or elevated uric acid levels after drinking coffee, monitor your condition and consult a specialist.

4. Prioritize measures that have been proven to reduce uric acid: In addition to coffee, medically proven strategies for controlling uric acid and reducing the risk of gout include:

  • Limit alcohol consumption, especially beer — this is a major factor in increasing uric acid levels.
  • Limit your intake of sugary/fructose-rich drinks and artificially sweetened soft drinks.
  • Control your weight, maintain a healthy lifestyle, drink enough water, and exercise regularly.

These recommendations are included in most clinical guidelines on the prevention and treatment of gout.
