Kidney failure at age 21

A 21-year-old man was hospitalized with generalized edema and a rapid weight gain of 5 kg, receiving a diagnosis different from the initial assessment.

At the age of 21, Mr. NBL (residing in Phung Nguyen commune, Phu Tho province) was diagnosed with stage 5 chronic kidney disease and advised to prepare for arteriovenous fistula (AVF) surgery to facilitate dialysis. However, after a re-examination and evaluation at Phu Tho Provincial General Hospital, doctors determined that the patient had acute kidney failure due to nephrotic syndrome, a condition that is potentially reversible with appropriate treatment.

According to the patient, about two months before hospitalization, he experienced generalized edema and gained about 5 kg. Because the symptoms did not significantly affect his daily life, he did not seek medical attention.

When he experienced severe fatigue and persistent nausea, he went to a major hospital in Hanoi for a check-up. There, he was diagnosed with stage 5 chronic kidney disease and advised to prepare for arteriovenous fistula creation for periodic dialysis.

Due to concerns about long-term treatment at a medical facility far from their residence, patients continue to visit the Kidney and Dialysis Center at Phu Tho Provincial General Hospital for examination.

Upon admission, the patient presented with mild edema in both legs, fatigue, nausea, poor appetite, chest pain during exertion, abdominal distension, and decreased pulmonary ventilation. Upon examination, the doctors found that some clinical manifestations did not entirely match a diagnosis of end-stage chronic kidney disease.

Patients undergo a comprehensive assessment of kidney function, body fluid status, and additional specialized tests to determine the cause of kidney dysfunction.

After consultation and monitoring, doctors determined that the patient had acute kidney failure due to nephrotic syndrome, not end-stage chronic kidney failure.

According to doctors, nephrotic syndrome can cause generalized edema, rapid weight gain, impaired kidney function, and affect many organs in the body. However, if the cause is correctly identified and treated promptly, kidney function can be restored.

The patient was then treated medically according to a professional protocol. After a period of treatment, urea and creatinine levels returned to normal limits, and kidney function recovered almost completely. Symptoms of edema, fatigue, and nausea also improved significantly. The patient did not need to undergo dialysis as initially planned.

According to Dr. Duong Thuy Linh from the Kidney and Dialysis Center, kidney diseases in young people can develop silently with symptoms such as edema, fatigue, abnormal urination, or rapid weight gain. Without early examination and accurate diagnosis, patients may miss the opportunity for effective treatment.

Doctors say that correctly identifying the cause of kidney dysfunction is crucial in choosing the right treatment method, avoiding unnecessary interventions, and improving the patient's chances of recovery.

Doctors advise people to visit specialized medical facilities when they experience symptoms such as facial swelling, leg swelling, decreased urination, persistent fatigue, loss of appetite, or unexplained rapid weight gain, in order to receive timely examination and treatment.
