Director Nguyen Quang Dung, 47 years old, experienced dull pain in his left chest. Doctors diagnosed him with acute myocardial infarction and promptly performed stent placement.

Arriving at the hospital in time, intervention within the "golden hour".

Earlier, director Quang Dung experienced pain and a burning sensation in his left chest, but he thought it was due to stress and excessive coffee consumption, so he didn't seek medical attention. The next morning, the chest pain returned, and he went to Tam Anh General Hospital in Ho Chi Minh City for a check-up.

On April 24th, Dr. Nguyen Van Duong, from the Interventional Cardiology Center at Tam Anh General Hospital in Ho Chi Minh City, stated that the patient exhibited symptoms of acute coronary syndrome. Electrocardiogram (ECG) and cardiac enzyme tests revealed acute myocardial infarction. The doctor activated the hospital-wide myocardial infarction emergency protocol.

According to Professor Vo Thanh Nhan, director of the Interventional Cardiology Center at Tam Anh General Hospital in Ho Chi Minh City, the patient arrived at the hospital in time and received emergency stent placement within the golden hour. "If intervention is delayed when the myocardium is severely necrotic, the patient is more likely to develop heart failure and arrhythmias later," Professor Nhan said.

Coronary angiography results revealed a complete blockage of one major blood vessel supplying the heart, a diffusely narrowed branch, and distal narrowing of the remaining branch.

Professor Nhan uses a new generation of stents coated with a special drug to prevent the growth of endothelial tissue. After stent placement, this drug is gradually released into the blood vessel, keeping the vessel wall smooth and reducing the rate of restenosis after intervention.

With the aid of intravascular tomography (OCT), doctors accurately measure the diameter of the blood vessel, select the appropriate stent size, expand it against the vessel wall, and quickly restore blood flow within 30 minutes.

The electrocardiogram results after the intervention showed no abnormalities, and the patient's heart rate was stable. The intervention was performed via the radial artery at the wrist, using local anesthesia, so the patient remained fully conscious throughout the procedure.

Rapid recovery

A few hours after the intervention, he was able to walk gently and experienced less chest pain. Three days after the intervention, he was discharged from the hospital. Half a month later, he returned to Tam Anh Hospital to have a second stent placed to widen the remaining two arteries in order to prevent the risk of recurrent myocardial infarction. Currently, his health is stable, and echocardiograms show good blood flow.

Director Quang Dung shared that he has a general health check-up every six months and exercises regularly, but in recent years, due to a busy work schedule, the frequency of check-ups has decreased, and he exercises less. He has smoked for many years and has an untreated lipid disorder.

"These are risk factors that accelerate the progression of atherosclerosis. The thickening of atherosclerotic plaques gradually narrows the blood vessels, causing severe narrowing and blockage of the coronary arteries, leading to acute myocardial infarction," Professor Nhan explained.

According to Professor Nhan, myocardial infarction is caused by the blockage or narrowing of one or more branches of the blood vessels supplying the heart. This is a common disease in people over 50 and is becoming more prevalent among younger people due to unhealthy diets, overweight and obesity, stress, and lack of exercise.

Besides a healthy lifestyle, proactive regular check-ups, early detection, and intervention help increase treatment effectiveness. With the advent of new-generation drug-eluting stents, patients with coronary artery stenosis have a greater chance of living healthier lives, extending their lifespan, and reducing the risk of undergoing more invasive procedures such as coronary artery bypass surgery.

The combination of drug-eluting stents with anticoagulants increases the effectiveness in preventing blood clot formation, reduces chest pain symptoms, and improves the quality of life for patients. However, restenosis of the coronary artery after stent placement can still occur, although at a very low rate.

Therefore, patients who have undergone intervention need to adhere to treatment, take medication regularly, and have regular follow-up appointments to promptly address any abnormalities and reduce the risk of restenosis of the coronary arteries.

Initially, the director was very scared when he heard about heart attacks, but his treatment experience at Tam Anh Hospital was very different.
