Proposal to amend regulations on training standards for the title of physician

(Chinhphu.vn) - The Ministry of Health is drafting a Circular regulating the standards for professional titles of doctors, preventive medicine doctors, and paramedics. In particular, the Ministry of Health proposes to amend the regulations on training standards for the title of paramedic.

The Ministry of Health proposed to regulate the training level of the title of grade IV physician as college physician, and rank civil servant salary A0.

The Ministry of Health said that in Article 10, Article 11 of Joint Circular No. 10/2015/TTLT-BYT-BNV stipulating the standards for training and fostering qualifications of the title of medical doctor grade IV, code V.08.03.07 requires the qualification: "Graduated from intermediate level medical doctor". The professional title of medical doctor is applied to the salary coefficient of type B civil servants, from salary coefficient 1.86 to salary coefficient 4.06.

On September 25, 2020, the Government issued Decree No. 115/2020/ND-CP regulating the recruitment, use and management of civil servants, replacing Decree No. 29/2012/ND-CP, which changes many regulations on civil servant management, thereby classifying training levels according to professional titles. Specifically, civil servants are classified according to training levels with the following professional titles: Civil servants holding professional titles requiring doctoral training levels; civil servants holding professional titles requiring master's training levels; civil servants holding professional titles requiring university training levels; civil servants holding professional titles requiring college training levels; civil servants holding professional titles requiring intermediate training levels.

Based on the complexity of the job title, civil servant job titles in the same career field are ranked from high to low as follows: a) Grade I job title; b) Grade II job title; c) Grade III job title; d) Grade IV job title; e) Grade V job title.

Based on the above regulations, the professional title of a grade IV physician requires intermediate level, and the salary classification of grade B civil servants is consistent with the classification of grade V civil servants.

On December 30, 2023, the Government issued Decree No. 96/2023/ND-CP detailing a number of articles of the Law on Medical Examination and Treatment. Clause 2, Article 8 stipulates: "A person with one of the following degrees is eligible to take the competency assessment test to grant a practice license for the title of medical practitioner:

a) Title of physician with scope of practice of general practitioner:

- College degree in general practitioner, including graduation diploma issued by a foreign educational institution recognized by the Minister of Labor, War Invalids and Social Affairs as equivalent to a college degree in general practitioner;

- Bachelor of Medicine degree issued by a foreign educational institution recognized by the Minister of Education and Training as a university degree.

b) Title of physician with scope of practice of traditional medicine physician: College degree of traditional medicine physician or college of traditional medicine, including graduation degree granted by a foreign educational institution recognized by the Minister of Labor - Invalids and Social Affairs as equivalent to college degree of traditional medicine physician or college of traditional medicine.

Clause 2, Article 127 stipulates: a) Title of physician with scope of practice of general practitioner:

- Intermediate medical degree, including graduation diplomas issued by foreign educational institutions recognized by the Minister of Labor, War Invalids and Social Affairs as equivalent to intermediate medical degree. These diplomas must be issued before January 1, 2027.

b) Title of physician with scope of practice of traditional medicine physician:

- Intermediate diploma in traditional medicine or intermediate diploma in traditional medicine, including graduation diplomas issued by foreign educational institutions recognized by the Minister of Labor, War Invalids and Social Affairs as equivalent to intermediate diploma in traditional medicine or intermediate diploma in traditional medicine. These diplomas must be issued before January 1, 2027.

Therefore, to match the professional titles of grade IV medical civil servants, in the draft, the Ministry of Health proposed to stipulate the training level of the title of grade IV medical doctor as college medical doctor, with salary classification of civil servants A0.

Medical officers who have been recruited and appointed as grade IV medical officers with intermediate training qualifications and grade B civil servant salaries before the effective date of this Circular must standardize their college training qualifications no later than January 1, 2030. In the case of officers who have a college medical degree, they will be classified as A0 civil servant salaries.

According to the draft, a Grade IV physician - Code: V.08.03.07 needs the following training and education standards: Graduated from college of general medicine or traditional medicine; have a license (certificate) to practice general medicine or traditional medicine or have a training certificate according to the professional title standards of a physician.

Standards on professional competence and expertise: Understanding the Party's viewpoints, policies and guidelines, the State's policies and laws on health care and improving people's health; determining disease status and factors affecting public health; detecting, intervening and preventing factors affecting public health.

In addition, have skills in organizing, implementing and monitoring primary health care and health management, occupational disease management; have skills in organizing, implementing and evaluating project activities, national target programs on health; have skills in cooperation and teamwork, coordinating with departments and organizations in performing tasks.
