Mr. Nguyen Van Hieu, Director of the Department of Education and Training of Ho Chi Minh City, reported the incident of the invigilator mistakenly signing the exam papers of candidates taking the 2024 graduation exam. According to the report, on June 27, during the Literature exam, at the Nguyen Du High School exam site, an incident occurred where the invigilator mistakenly signed the signature box of the examiner on the exam papers of all candidates in room 2,500.
Specifically, Mr. LQH - a teacher at Hoang Van Thu Secondary School (District 10), was assigned to be the invigilator of exam room 1 - exam room 2,500, and mistakenly signed in the signature box of the examiner on the test papers of 22/22 candidates. Exam room 2,500 had 22/24 candidates taking the exam, including students from Nguyen Du High School and Suong Nguyet Anh High School.
At 7:40 a.m., when he discovered the error, Mr. LQH arbitrarily distributed new exam papers to all candidates in the room and asked the candidates to copy their answers onto the new papers. At 7:45 a.m., due to disagreement on how to handle the situation, the second invigilator in room 2,500 reported the incident to the exam room supervisor outside and the incident was reported to the head of the exam site.
Mr. NTT - Principal of Dien Hong High School - Head of Nguyen Du High School examination site assigned Ms. LTXC - Head of Professional Group of Nguyen Hien High School, Deputy Head of Examination Site, to go directly to the examination room, request the invigilator of examination room 2,500 to handle the matter on the principle of ensuring the highest benefits for candidates, and at the same time make an unusual record of the incident to report to the examination council.
However, the head and deputy head of the examination site did not check and grasp the actual situation in the examination room, did not supervise the invigilators in carrying out the instructions of the head of the examination site, resulting in a situation where some candidates took the test on new exam papers, while others continued to take the test on old exam papers.
The Director of the Department of Education and Training of Ho Chi Minh City said that upon receiving the report, the Standing Committee of the Steering Committee for the 2024 Ho Chi Minh City High School Graduation Exam directed the examination council to review the Literature exam of all candidates in room 2,500. Through the review, it was found that all candidates completed the exam.
Two invigilators of the Literature exam in room 2,500, Mr. LQH - teacher at Hoang Van Thu Secondary School (District 10) and Mr. KTH - teacher at Nguyen Van To Secondary School (District 10), head and deputy head of the examination site at Nguyen Du High School, were disciplined for their responsibilities.
The examination board established a team to analyze the results of the Literature test of candidates in room 2,500 based on comparison with the general results of candidates in the whole city, assess the impact of the incident on candidates in this room, and ensure the rights of candidates.
