Girl with acute myocarditis almost died

Thua Thien - Hue An 11-year-old girl suffered from cardiogenic shock and ventricular fibrillation due to acute myocarditis. After 6 days of ECMO intervention, she escaped critical condition.

On June 9, a representative of Hue Central Hospital said that the child was admitted last week with pain behind the sternum, and an electrocardiogram showed ventricular arrhythmia and myocardial ischemia. Test results showed damage to the heart muscle. The patient was diagnosed with acute myocarditis, with a very high risk of death.

After a day of treatment, the patient became more serious, with ventricular fibrillation, decreased consciousness, and no pulse. Doctors performed cardiopulmonary resuscitation, gave electric shock twice, and administered intravenous antiarrhythmic drugs, but the ventricular arrhythmia persisted. The child continued to suffer from shock, with low blood pressure, no urine output, and reduced myocardial contractility to only 32%.

Doctors determined that this was a case of cardiogenic shock due to acute myocarditis with ventricular fibrillation, with a 100% mortality risk. After a hospital-wide consultation, the baby was treated with extracorporeal membrane oxygenation (ECMO) combined with invasive mechanical ventilation and intensive medical treatment.

Fortunately, after 6 days, the heart's contractile function improved, myocardial damage decreased, vital function parameters improved, and the patient was weaned off ECMO and the ventilator. Currently, the patient is alert, responsive, has stable pulse and blood pressure, and does not need respiratory support. He is expected to be discharged early next week.

It is not yet clear what caused the baby's fulminant myocarditis.

Prof. Dr. Pham Nhu Hiep, Director of Hue Central Hospital, said that ECMO technique was successfully performed by doctors at Hue Central Hospital in March 2009, mainly in resuscitation of patients with myocarditis and cardiogenic shock after cardiovascular surgery.

The baby above is the first case to receive ECMO intervention at the hospital's Children's Center.
