What causes dizziness during perimenopause?

Dizziness in menopause is primarily due to a decrease in estrogen levels, disrupting the autonomic nervous system, altering blood flow, or affecting the inner ear and vestibular system. From age 40 onwards, the ovaries begin to decline in function, reducing estrogen production, leading to hormonal imbalances and other disorders in the body. Estrogen levels affect the functioning of the inner ear, a crucial part of maintaining the body's balance.

Furthermore, the female hormone estrogen also affects the neurohumoral balance of the body's circulation. Therefore, hormonal imbalance affects the body's ability to stabilize the vestibular system, causing dizziness and loss of balance.

Hormonal imbalances in women can be accompanied by other contributing factors such as: prolonged sleep disorders, or poor sleep quality. Prolonged sleep disorders are a common cause of benign positional vertigo in the morning and can also increase the risk of cardiovascular disease (high blood pressure), worsening vestibular disorders and leading to more frequent and severe vertigo.

In addition, work pressure, family pressure, or stress and anxiety due to declining bodily functions can also increase the frequency of dizziness.

Symptoms of premenopausal dizziness

In some women, menopausal symptoms are mild, while others (around 20%) experience various disorders, particularly symptoms of perimenopause such as hot flashes, insomnia, weight gain, joint pain, vaginal dryness, menstrual irregularities, and psychophysiological disturbances. 90% of menopausal women experience sudden episodes of dizziness that become increasingly frequent and severe.

Initially, symptoms such as headaches, dizziness, and a feeling of imbalance occur infrequently and are transient, resolving with rest. However, they become more frequent and severe over time, most notably manifesting as a spinning sensation—this is the symptom of "true vertigo" experienced by menopausal women, but it is benign positional vertigo.

Besides dizziness and disorientation, menopausal women may experience accompanying symptoms such as lightheadedness when changing positions suddenly (lying on the left or right side, standing up or sitting down, bending down, or looking up...). In addition, menopausal women may also experience symptoms such as persistent tinnitus affecting hearing or visual disturbances (blurred vision or seeing unusual flashes of light...), and often feel tired and less agile.

Principles of treating dizziness in premenopausal women.

Regardless of gender or age, when experiencing dizziness, it is advisable to seek medical attention at a qualified healthcare facility to determine the cause. Based on this, a doctor will prescribe appropriate treatment. Self-diagnosis and self-treatment should be avoided.

For dizziness caused by perimenopause in women, treatment options include hormone replacement therapy, and if necessary, specific medications should be prescribed by a doctor, especially a specialist.

What can be done to reduce dizziness in premenopausal women?

People in the menopause age group should maintain a healthy diet, increasing their intake of green vegetables, seafood, and omega-3 rich nuts; limit or avoid alcohol and smoking. They should try to get enough sleep and avoid unnecessary stress to maintain a stable nervous system and prevent exhaustion. Regular daily exercise such as walking, yoga, swimming, or playing badminton helps improve blood circulation.
