Upon taking the patient's medical history, it was noted that Mr. V. had a prior history of pulmonary tuberculosis. After examination and a chest CT scan, a large air cyst was found in the upper lobe of the right lung that had ruptured, causing a large amount of pneumothorax in the right pleural space. Simultaneously, this resulted in partial passive collapse of the parenchyma in the lower right lobe and displacement of the mediastinum and cardiac shadow to the left, presenting a critical condition. Doctors from the Emergency, General Surgery, and Intensive Care units quickly consulted and decided to perform continuous pleural drainage.

On September 12th, Dr. Nguyen Thanh Sang, Deputy Head of the General Surgery Department at Xuyen A General Hospital in Tay Ninh , stated that the patient subsequently developed pneumothorax due to a ruptured air cyst. Conservative treatment was ineffective, with air continuing to leak and requiring drainage after 48 hours. Therefore, the doctors unanimously decided to perform thoracoscopic surgery to remove the air cyst in the right lung. During the surgery, the team successively removed the cluster of air sacs at the apex of the right lung, sutured the leaking lung parenchyma, caused adhesion of the right pleura, and improved the patient's respiratory function.
Following the surgery, the patient's shortness of breath has decreased, and their health is gradually recovering well.
According to Specialist Doctor Nguyen Thanh Sang, pulmonary air cysts are abnormal air sacs that form in the lungs due to the destruction and expansion of alveoli. Large air cysts can occupy a lot of space, reducing gas exchange and causing shortness of breath. More dangerously, when an air cyst ruptures, the patient may experience pneumothorax, leading to lung collapse and acute respiratory failure. If not treated promptly, this condition can be life-threatening.
Endoscopic resection of pulmonary cysts is a minimally invasive treatment method that offers many advantages over traditional methods, such as smaller incisions, less pain, reduced intra- and post-operative complications, faster patient recovery, and shorter treatment time and costs.
