On the afternoon of October 4, information from the People's Committee of Dray Bhang commune said that the People's Committee of the commune had just sent a quick report to the People's Committee of Dak Lak province about the car crash that occurred on the evening of October 3, causing the death of the Chairman of the People's Committee of the commune, Le Phuoc Toan, while on duty. The report also requested the People's Committee of the province to agree on the policy so that the People's Committee of the commune can complete the procedures to submit to the competent authorities for recognition of martyrs.
According to the report of the People's Committee of Dray Bhang commune, at around 6:00 p.m. on October 3, heavy rain occurred in Dray Bhang commune, causing localized flooding in some locations. In particular, at Km 16+650 on National Highway 27, passing through Hamlet 4, Dray Bhang commune, the flooding was severe, causing traffic congestion and endangering vehicles and pedestrians.

Upon receiving the news, comrade Le Phuoc Toan, Deputy Secretary and Chairman of the People's Committee of Dray Bhang commune, along with officers and soldiers of the commune police and local authorities, quickly arrived at the scene to assist in traffic flow, warn of danger, and ensure safety for people and vehicles.
At the scene, comrade Le Phuoc Toan as well as the functional forces used flashlights to warn and direct traffic. However, at about 9:25 p.m., while comrade Le Phuoc Toan was standing close to the edge of the road, warning of traffic danger, a 7-seat car with license plate 47A-461.73 driven by female driver Le Thi Cam Chi (born in 1992, residing in Ea Ktur commune, Dak Lak province) traveling at high speed from Ea Ktur commune to Buon Ma Thuot ward hit him head-on.

The strong impact caused comrade Le Phuoc Toan to fly dozens of meters away. Immediately after, people urgently took the victim to the Central Highlands General Hospital for emergency treatment, but due to the severe trauma, comrade Le Phuoc Hoan died at the hospital.
Discussing the proposal to recognize comrade Le Phuoc Toan as a martyr, Chairman of Dak Lak Provincial People's Committee Ta Anh Tuan said that the province is asking the Department of Home Affairs to review the best policies for the family and comrade Le Phuoc Toan, affirming that the province will pay close attention.
“The unexpected sacrifice of comrade Le Phuoc Toan, a cadre who always devoted himself to the people and took his responsibilities seriously, has left behind deep sorrow and is a shining example of the spirit of “serving the people” for cadres and party members to follow,” said the Chairman of the Dak Lak Provincial People's Committee.
