Citizen identification card (CCCD) or valid ID card, photocopy 2 copies and bring the original. In case you are using a CCCD with chip, you do not need to provide any additional documents, otherwise, you must attach a certificate of residence information or a personal identification number notification issued by the local police.

Pregnancy examination records include pregnancy examination book, ultrasound results, tests done during pregnancy... In particular, in cases where the pregnant mother has accompanying diseases such as cardiovascular disease, diabetes, hepatitis..., she needs to bring examination records of these specialists.

Health insurance (HI) and original HI referral form (if any). You can present your HI by presenting your CCCD with chip or original HI card or through the VssID application - social insurance.

According to the new regulations added on January 1, 2021, in case you have health insurance but are hospitalized without a referral, you will still be entitled to the same benefits as the correct route with 2 conditions: participating in health insurance for 5 consecutive years; the amount of health insurance paid in the year does not exceed 6 months of basic salary.

In case you have other types of service insurance cards, you need to contact the insurance company to get information about whether your insurance contract is directly guaranteed at the hospital or not and specifically how your costs will be guaranteed before going to the hospital. When admitted to the hospital, you need to present the original insurance card and CCCD. If the insurance you purchased is on the list of companies affiliated with the hospital, the staff in charge will guide you through the procedure.

Note that if you have both health insurance and service insurance, you can only choose one of the two types of insurance to receive at the hospital, the remaining type of insurance you will pay yourself at the insurance agency.
