Secondary infertility due to uterine fibroids
Uterine fibroids are benign tumors located in the muscle or wall of the uterus. The severity of the disease depends on the number, size and location of the tumors.
There are cases where the disease does not cause any symptoms. However, there are many cases that cause complications such as menstrual disorders, lower abdominal pain, and affect fertility.
The case of Ms. Thanh Lan (43 years old, Hanoi , character name has been changed) is an example. More than 10 years ago, she was diagnosed with uterine fibroids. Not only did she have to live with the uncomfortable symptoms of the disease, she also suffered from secondary infertility.
She has had in vitro fertilization (IVF) 5 times at different medical facilities but has never been successful.

Recently, she was prescribed US-HIFU treatment to effectively treat the tumor while preserving the uterus and maintaining her inherent fertility. This is a technique for treating tumors using high-intensity focused ultrasound waves under ultrasound guidance.
Just one month after uterine fibroid treatment, Ms. Lan got pregnant naturally without any reproductive support measures, something she never dared to think about before.
Professor - Doctor Nguyen Viet Tien, former Deputy Minister of Health , Director of Thien An Obstetrics Hospital, said that the most important thing is that the patient retains the uterus and maintains the ability to give birth.
In addition, the uterus is not incised like in open or laparoscopic surgery, avoiding weakening of the uterine muscles. In some cases during pregnancy, the enlarged uterus can lead to uterine rupture.
On December 27, 2024, the hospital was permitted by the Ministry of Health to deploy the technique of treating tumors with high-intensity focused ultrasound waves under ultrasound guidance (US-HIFU).
To date, the initial preliminary report shows that all patients after treatment have no complications and recover quickly. The results of the re-examination after one month of the patients show that the tumor volume has significantly decreased by more than 60% for uterine fibroids and more than 80% for uterine adenomyosis.
Symptoms of abdominal pain, frequent urination, difficulty urinating, menorrhagia, metrorrhagia... caused by the large tumor before were also greatly reduced.
Infertility treatment needs to prioritize treating the root cause
Widely applied in many developed countries around the world for more than 20 years, this technology has been recognized as a breakthrough in the treatment of benign and malignant tumors such as liver tumors, pancreatic tumors, breast tumors, uterine fibroids, endometriosis, etc.
US-HIFU uses the penetrating and converging properties of ultrasound waves to directly focus on target tissues in the body and increase the temperature up to 100 degrees Celsius, causing protein denaturation and irreversible coagulation necrosis of the target tissue, disrupting the blood supply to the pathogenic cells. The tumors will then gradually disappear within 18 to 24 months.

The treatment process does not require deep anesthesia, the patient remains awake, there is no bleeding, no pain, and can be discharged the same day.
According to Professor Tien, infertility has many causes and in vitro fertilization is not the only way to treat it. Infertility due to infection must be treated to completely eliminate the infection; infertility due to blocked fallopian tubes, old cesarean section scars, abnormalities in the reproductive organs, etc., require surgery to correct.
After treatment, patients can get pregnant naturally without the need for assisted reproductive measures.
According to him, to improve treatment effectiveness, both husband and wife need to be carefully examined, have their medical history taken, find the main cause and thoroughly treat it with medical treatment methods, open surgery, laparoscopic surgery, US-HIFU treatment... before proceeding to IVF.
