Rare case: 3cm fish bone pierces bile duct.

On the afternoon of March 26th, Can Tho Central General Hospital announced that doctors from the Endoscopy Department had successfully removed a 3cm long fish bone that had pierced the stomach and entered the bile duct, disrupting bile flow and causing cholangitis and prolonged abdominal pain, using endoscopic retrograde cholangiopancreatography (ERCP).

Previously, male patient DNHĐ (born in 1981, residing in Vinh Thanh commune, Can Tho city) was admitted to the hospital with pain in the epigastric and right hypochondriac regions, accompanied by jaundice and yellowing of the eyes. The patient stated that this abdominal pain had lasted for many months and had not been relieved by examinations at many places. At Can Tho Central General Hospital, the patient was admitted, monitored, and treated in the General Surgery Department.

Upon examination and performing paraclinical tests, the doctors noted dilation of the intrahepatic bile ducts, abnormalities in the bile ducts in the hepatic hilum, and stricture of the two hepatic branches leading to the common hepatic duct. Notably, the CT scan revealed a radiopaque foreign body approximately 3 cm long in the hepatic hilum, suspected to be the cause of the bile duct obstruction.

After a multidisciplinary consultation, the team decided to perform ERCP (Endoscopic Retrograde Cholangiopancreatography) to investigate and remove the foreign body, performed by Dr. Nguyen Thi Quynh Mai, Head of the Endoscopy Department. During the endoscopy of the bile duct, a fish bone was found penetrating the common hepatic duct from the outside, with a stone lodged above it. The doctors successfully removed the 3cm fish bone using forceps under Spyglass guidance and placed a stent in the right hepatic branch to relieve the bile duct obstruction. Currently, the patient is conscious, responsive, and experiencing significant improvement in abdominal pain and reduced jaundice. Discharge is expected in the next few days.
