Insomnia and chest tightness when sleeping are heart disease?

Recently I have been having insomnia for several days in a row, and when I have insomnia I feel tightness in my right chest and pain in my right shoulder, it feels like there is a nerve running straight from my shoulder down to my right nipple and back, a slight throbbing pain and a cold feeling.
