The case of Mr. NVT (25 years old, Hanoi ) is a typical example. Mr. T. came to Medlatec General Hospital after feeling pain in his right buttock for the past 2 months. The pain symptoms appeared more clearly when sitting for a long time, accompanied by a feeling of numbness in his right foot.
After the examination, MSc.BSNT Le Thi Duong, a specialist in Musculoskeletal System at Medlatec, conducted the necessary tests.

The results showed that Mr. T. had pain in the right hip defect. Based on the MRI results, the doctor determined that he had piriformis syndrome - a rare disease of the gluteal muscle group.
MRI showed that Mr. T.'s L5/S1 disc was herniated, causing spinal stenosis, and the right piriformis muscle was 19mm thick (compared to 12mm on the left). This condition led to muscle hypertrophy, compressing the sciatic nerve, causing pain.
The piriformis muscle is a small muscle located deep in the buttocks, which plays an important role in rotating the hips and lifting the legs. When this muscle is swollen and contracted, it causes piriformis syndrome, compressing the sciatic nerve, leading to symptoms such as: Pain in the buttocks radiating down the legs, especially when sitting for a long time.
Pain when moving the hip joint, especially when bending, closing or rotating the hip joint, numbness in the legs: Feeling of numbness and weakness in the legs when the sciatic nerve is compressed.
This syndrome can have many causes, including damage to the hip joint, buttocks due to an accident or strong impact. Or due to prolonged sitting, especially common in office workers, drivers or cyclists.
Overexertion: Especially for people who exercise excessively such as weightlifting or have anatomical abnormalities where the piriformis muscle splits in two, causing compression of the sciatic nerve.
According to MSc. Dao Danh Vinh, Director of Medlatec Imaging Diagnostic Center, MRI is an important tool in diagnosing piriformis syndrome, helping to detect inflammation, muscle hypertrophy, disc herniation and other diseases in the coccyx area.
Mr. T's case showed obvious signs of hypertrophy with a right piriformis muscle thickness of up to 20mm, a typical sign of this syndrome.
Depending on the severity of the disease, the doctor will recommend different treatment methods, including medical treatment: Limiting painful movements.
Physical therapy: Simple stretching exercises, combined with heat, shortwave or electrical pulse therapy to relieve pain.
Medication: Use pain relievers, nonsteroidal anti-inflammatory drugs, or steroid and botulinum toxin type A injections to reduce muscle spasms.
Surgery: Surgery is indicated if the above measures are not effective, to relieve nerve compression.
To prevent piriformis syndrome and musculoskeletal diseases, doctors recommend that people maintain correct posture, avoid sitting for long periods of time, and maintain a comfortable sitting position.
Warm up thoroughly before exercising: This helps the body adapt easily and avoid injury. Exercise moderately, increase the intensity of exercise gradually, do not overdo it suddenly.
Regular health care: Especially for office workers or those at high risk, should have early check-ups when there are unusual signs for timely detection and treatment.
Mr. NVT's case is a reminder of the importance of paying attention to musculoskeletal health, especially for those who have a habit of sitting for long periods or are sedentary.
