Surgery to remove a rare 9kg sarcoma tumor.

Hanoi Oncology Hospital has successfully performed surgery to remove a rare, enormous uterine sarcoma tumor that occupied almost the entire abdominal cavity, compressing multiple internal organs, respiratory system, and daily activities of the patient.

Patient PHL (female, 45 years old, residing in Hanoi ) was previously completely healthy, with no detected medical conditions and was unmarried.

Approximately six months before hospitalization, Ms. L. began experiencing dull abdominal pain, mainly at night, accompanied by weight loss and wasting. However, due to a complacent attitude and embarrassment, she did not want to go to the hospital for examination.

In recent months, the patient's condition has worsened significantly, with the abdomen becoming increasingly distended, pains becoming more frequent, appetite decreasing, and shortness of breath progressively increasing. The patient cannot lie on their back and can only lie on their side due to the feeling of being compressed.

When the symptoms became severe, Ms. L. finally went for a check-up and was found to have a large tumor in her abdomen.

The CT scan revealed a diffuse tumor in the right hypochondrium, periumbilical region, and hypogastric area, measuring up to 22.7 x 29 cm with irregular borders. The tumor was in contact with and compressing multiple organs such as the liver, gallbladder, right kidney, and pancreas, while also adhering to the uterine fundus. Additionally, the patient presented with significant ascites and bilateral pleural effusion.

Due to the tumor growing large and causing severe compression, the doctors held a consultation and decided to perform semi-emergency surgery on the patient.

During surgery, the tumor was identified as originating from the uterine fundus, measuring nearly 30cm at its largest point, occupying almost the entire abdominal cavity, and adhering to the colon, the mesentery of the small intestine, and many surrounding structures.

The surgical team performed a complete removal of the tumor along with the entire uterus and both adnexa to ensure maximum removal of the lesion. The surgery lasted approximately 2 hours, during which the patient received 4 units of blood and intensive care. The removed tumor weighed nearly 9 kg and measured approximately 30 x 17 cm.

Following the surgery, the patient's condition improved, the surgical wound was dry, their health was stable, they could eat, drink, and walk normally, and were discharged after 7 days.

Postoperative pathology results confirmed a diagnosis of stage III uterine leiomyosarcoma. The patient was advised to continue adjuvant chemotherapy to reduce the risk of recurrence.

According to Dr. Le Van Thanh, Deputy Director of Hanoi Oncology Hospital and Head of the General Surgery Department, this is a typical case of neglecting initial symptoms, allowing the tumor to develop silently and progress to a late stage, potentially leading to many dangerous complications and even threatening the patient's life.

Unfortunately, to ensure complete removal of the tumor and patient safety, doctors were forced to perform a total hysterectomy, affecting future fertility if the patient still wishes to have children.

Doctor Thanh advises people not to ignore unusual signs such as prolonged abdominal pain, rapid abdominal enlargement, poor appetite, or difficulty breathing.

Early diagnosis helps detect the disease at an treatable stage, thereby improving treatment effectiveness, reducing surgical invasiveness, and increasing the chances of organ preservation.
