The England midfielder had to leave the field on a stretcher after a knee collision in Tottenham's friendly with Newcastle last week.
James Maddison has undergone a number of medical tests in London and initial fears of cruciate ligament damage have come true.

This is a double nightmare for new coach Thomas Frank, as he will be without Maddison for most of the upcoming 2025/26 season.
A statement on Spurs' website read: "James Maddison will undergo surgery to repair a ruptured anterior cruciate ligament (ACL) in his right knee.
The 28-year-old midfielder suffered the injury during a pre-season friendly against Newcastle in Seoul. Surgery will be carried out shortly and James will begin his rehabilitation with the Spurs medical team.
Everyone wishes James a speedy recovery. We will continue to support him as he continues his treatment and recovery from surgery."
James Maddison's injury has forced Tottenham back into the transfer market, despite signing Mohamed Kudus from West Ham for £55m.
The Europa League champions also missed out on Morgan Gibbs-White despite being ready to trigger his release clause. Nottingham Forest reacted angrily and threatened legal action against Tottenham over their illegal approach.
