This is one of the rare hymen abnormalities in children, parents should absolutely not be subjective.
One day before being admitted to the hospital, a girl named NMT (14 years old, Hanoi ) experienced lower abdominal pain. The pain was described as a constant dull abdominal pain and pain in the lumbar region. The pain did not subside, so her family took her for a check-up.
The doctor conducts a clinical examination and prescribes the necessary paraclinical techniques for diagnosis.
Through questioning, it was learned that T. had not had her period yet. A gynecological examination revealed that her hymen was not perforated.
In particular, the ultrasound results showed that in the uterine cavity there was 11mm thick fluid, 9mm thick mucosa. The vaginal location had heterogeneous negative fluid, communicating with the fluid in the uterine cavity measuring 74x76x157mm.
The doctor diagnosed the patient's condition as amenorrhea due to an imperforate hymen, specifically the child had menstruation but it did not come out, causing pain in the lower abdomen. T was advised to have surgery to cut the hymen to let the menstrual blood out.
Imperforate hymen - a rare defect with an incidence of 1/1,000
Dr. Nguyen Hai Duyen - Pediatrics (Obstetrics), Medlatec General Clinic - the doctor who directly examined T.'s case said that amenorrhea due to an imperforate hymen is a condition of menstrual blood stasis in the uterus - vagina due to the hymen not having a hole, causing menstrual blood to not escape and stagnate in the uterine cavity, vagina, and can overflow through both sides of the fallopian tubes into the abdomen. This is a rare malformation, with an incidence rate of 1/1,000.
If this condition is not detected and treated promptly, it can cause dangerous complications such as bilateral fallopian tube hematoma, fallopian tube rupture, uterine and fallopian tube infection due to menstrual blood stasis creating conditions for bacteria to grow, affecting fertility.
However, according to Dr. Duyen, diagnosing amenorrhea due to an imperforate hymen is relatively simple, based on clinical and some paraclinical techniques as follows:
Ultrasound is an initial diagnostic imaging method, easy to perform, and has high diagnostic value;
Magnetic resonance is a modern method that helps to confirm diagnosis, especially in cases with atypical ultrasound and cases with complications to provide appropriate treatment.
