E Hospital successfully performed its first emergency autologous kidney transplant.

A 31-year-old male patient who suffered particularly severe kidney damage after a traffic accident had his kidney saved by doctors using an autologous kidney transplant instead of having it removed.

Recently, doctors from the Department of Urology and Andrology, in collaboration with the Cardiovascular Center of E Hospital, successfully performed an emergency autologous kidney transplant on a 31-year-old male patient from Hanoi who suffered multiple severe injuries in a traffic accident, saving a kidney that was previously thought beyond repair.

According to the patient's medical history, the patient was involved in a traffic accident in the early morning of May 20th and was admitted to E Hospital with very severe injuries including blunt chest trauma, blunt abdominal trauma, grade 3 liver rupture, and grade 5 kidney injury.

Upon emergency examination, doctors determined that the liver and chest injuries did not require immediate surgical intervention, while the right kidney was in critical condition due to complex damage at the renal hilum, severed vascular pedicle, and renal vein thrombosis.

Dr. Nguyen Dinh Lien, Head of the Department of Urology and Andrology, said this was an exceptionally difficult case with the risk of complete kidney dysfunction, severe bleeding, and life-threatening hemorrhagic shock. Immediately, the hospital-wide red alert system was activated, with consultations involving multiple specialties to find the optimal treatment plan.

According to doctors, in cases of grade 5 kidney injury and ruptured blood vessels that arrive at the hospital late, the often-chosen solution is to remove the damaged kidney to save the patient's life. However, this means the patient loses half of their dialysis function for life.

After interdisciplinary consultation, the team decided to perform autologous kidney transplantation – a technique that involves removing the patient's own kidney from the body, treating the damage, and then transplanting it back in.

Under the leadership of Dr. Phan Thao Nguyen, Deputy Director of E Hospital and Head of the Cardiovascular Center, the doctors chose to transplant the kidney back into the original renal fossa instead of moving it down to the pelvic fossa as is typically done.

This method helps patients avoid an additional incision and reduces complex manipulations on the ureter, but it requires advanced technique due to the narrow and deep surgical field, making vascular anastomosis particularly difficult.

After many hours of surgery, the team completed the microsurgical sutures and proceeded to unclamp the blood vessels. Blood flow returned, the kidney gradually recovered and began to excrete urine, indicating that kidney function had been restored.

More than 10 days after surgery, the patient's condition is recovering well, with normal eating and walking. Biochemical test results and imaging studies show that kidney function has returned to normal and renal blood vessels are functioning well. The patient is expected to be discharged in the coming days.

Doctors said this was the first autologous kidney transplant performed at the hospital. The success of the surgery helped preserve the patient's kidney and demonstrated the effectiveness of interdisciplinary collaboration in managing cases of severe multiple trauma and complex kidney injuries.
