In Document No. 5851/BYT-TCCB dated August 29, 2025 signed by Minister of Health Dao Hong Lan, it is stated that, implementing Conclusion No. 137-KL/TW of the Politburo and Secretariat on the Project to rearrange and reorganize administrative units at all levels and build a 2-level local government organization model, for the organizational model of commune health stations and district health centers: "Maintain the educational and health service units and transfer to the commune-level government the management of junior high schools, primary schools, kindergartens, and health stations of the new grassroots units to ensure health care for people right at the grassroots level and ensure enough schools and classes for students in the grassroots area.
For medical centers under the People's Committees at the district level, they will now be transferred to the Department of Health for management to organize service provision according to inter-commune and ward areas.
However, in reality, some localities have implemented the merger of commune health stations. Regarding the Circular guiding the functions, tasks, powers and organizational structure of commune, ward and special zone health stations according to the 2-level local government organization model, the Ministry of Health said that the Ministry is currently drafting and seeking comments from ministries, branches and localities for promulgation.
Speaking to reporters from the Government Electronic Newspaper, a representative of the Department of Organization and Personnel, Ministry of Health, said that the Ministry of Health has drafted a Circular guiding the functions, tasks, powers and organizational structure of commune, ward and special zone health stations according to the 2-level local government organization model. However, the current difficulty is choosing an operational model.
The instructions and functions of commune health stations are also very new, and there are still different opinions on many contents.
Previously, readers of the Government Newspaper reported that the lack of a circular guiding the functions, tasks, powers and organizational structure of commune, ward and special zone health stations under the 2-level model has made it difficult to advise the People's Committees of new communes and wards in the health sector.
On June 23, 2025, the Ministry of Health issued Circular No. 20/2025/TT-BYT guiding the functions, tasks, and powers of specialized health agencies under the People's Committees of provinces and centrally run cities and the People's Committees of communes, wards, and special zones under provinces and centrally run cities.
Deploying 1000 doctors to the grassroots level
Also in document No. 5851/BYT-TCCB, implementing the Prime Minister's direction in document No. 01-TB/DU dated April 2, 2025 on paying attention to preventive medicine and grassroots health care, especially in the context of merging a number of provincial-level administrative units, not organizing at the district level, to stabilize human resources to serve the care, protection and improvement of people's health well, towards all people being equal in enjoying convenient and quality medical services from the central to local levels, it is necessary to urgently deploy 1,000 doctors to the grassroots level, the Ministry of Health said that the Ministry is currently developing a project to implement this content.
To ensure that medical services at the grassroots level are not interrupted and that people have access to quality, convenient and effective medical services, the Ministry of Health recommends that the People's Committees of provinces/cities directly under the Central Government and the Department of Health of provinces/cities directly under the Central Government have plans to rotate, transfer and mobilize human resources from the Department of Health, hospitals and medical centers under the Department of Health, and related units under the People's Committees of provinces/cities to the commune level (including at least 1 medical specialist at the Department of Culture and Society and 1 doctor at the commune health station).
According to the leaders of the Ministry of Health, this is to ensure that the operations of the grassroots health system are not interrupted, providing full professional and technical services to meet the model of a two-level local government organization system.
