High fever, body stiffness due to heat shock after many hours of working outdoors in the sun

After working outdoors in the scorching heat, a male construction worker was rushed to the hospital in critical condition. When his family took him to the hospital, he was in a state of total body stiffness, biting his tongue causing bleeding from his mouth and had a fever of 42 degrees Celsius.

The male patient (42 years old, from Bac Ninh) was transferred to the Emergency Department, Central Hospital for Tropical Diseases on June 2 (the day Hanoi and surrounding areas were at their peak of heat) in critical condition.

Patient TVT, a construction worker, began to feel exhausted around 15–16 hours after many hours of continuous work under the sun. He became lethargic and uncommunicative, then developed abnormal symptoms such as convulsions, impaired consciousness, and loss of behavioral control.

When taken to the medical facility by his family, the patient was in a state of total body stiffness, biting his tongue causing bleeding in the mouth and had a high fever of 42 degrees Celsius.

According to Master, Doctor Truong Tu The Bao, the doctor directly treating the patient, the patient was hospitalized with typical signs of severe heat stroke, including hyperthermia, central nervous system disorders such as convulsions, drowsiness, coma; accompanied by electrolyte disturbances and acute liver and kidney damage.

Notably, the patient has a history of alcohol abuse and cirrhosis - these factors make the patient susceptible to heatstroke and the condition when suffering from heatstroke and sunstroke also progresses more severely and recovery is slower than normal.


Doctor Bao said that tests showed elevated liver enzymes, clear signs of liver damage, most likely due to the simultaneous effects of alcohol and heat shock.

According to the patient's family, he had a habit of drinking alcohol regularly, although his health had declined significantly recently. Alcohol abuse not only causes damage to the liver but also reduces the body's ability to adapt to heat, making the body susceptible to collapse when exposed to extremely hot weather.

Currently, patient T. has overcome the critical stage, is being closely monitored at the hospital and actively treated with measures such as fluid infusion, electrolyte adjustment, liver and kidney function support... Biochemical indicators will continue to be monitored to assess the ability to recover in the coming days.

According to Dr. Bao, heat stroke is the result of the body accumulating heat due to prolonged hot weather, when the natural thermoregulation mechanisms no longer function effectively.

There are two common mechanisms: one is due to direct sunlight on the head and neck, which can cause central nervous system disorders, leading to drowsiness, fainting or even death.


Second, the hot environment due to prolonged exposure to high environmental temperatures makes the body unable to eliminate heat in time, leading to electrolyte disturbances, circulatory failure and damage to multiple organs such as the liver, kidneys and brain.

Initial symptoms of heat stroke include fatigue, dizziness, nausea, headache, rapid breathing, high body temperature, lethargy, and convulsions. If not detected and treated promptly, the patient can quickly fall into a deep coma, acute liver failure, muscle necrosis, multiple organ failure, and death.

Doctors recommend that outdoor workers should arrange reasonable rest time, especially avoiding working during the hot sun hours from 10am to 4pm.

When working, you should wear a wide-brimmed hat, long-sleeved clothes, use a scarf to protect your neck, and drink enough water and regularly replenish electrolytes to avoid dehydration and body temperature disorders.

When unusual signs such as fatigue, dizziness or headache appear, you should immediately stop working, find a shady place to rest and closely monitor your health. For people with chronic diseases such as cirrhosis, kidney failure... or who are abusing alcohol, the risk of heat stroke and serious complications will be much higher.


People need to be especially careful when working in hot weather conditions. If you suspect someone has sunstroke or heatstroke, quickly move the victim to a cool place, lower the body temperature by wiping the body with a warm towel, fanning gently, giving water if conscious and quickly transporting to the nearest medical facility for timely treatment.
