The most recent case was a patient named TT, 35 years old, who went to the emergency room due to her menstruation lasting nearly a month, causing her to become dizzy. The ultrasound results showed fluid retention in the uterus, vaginal bleeding, and blood clots.
Born female, but since childhood T. liked to have short hair and play with boys. T. has been taking hormone injections to change her external appearance to that of a man (from female to male) since 2023.
The drug has the role of supplementing Testosterone to enhance male characteristics, inhibiting the body from reducing the production of estrogen - a hormone related to the development of female characteristics. After 2 months of injection, Tu's voice began to change, after about 4 months his face became more masculine, and he began to grow a beard on his upper lip and chin.
At first, after injecting the male hormone Testosterone, T. felt comfortable when her period stopped completely. After nearly a year of using the drug, she had her period twice a month, with a heavy flow, making T. tired and pale. The patient went to see a gynecologist and took medication, but the bleeding did not improve.
Doctor My Nhi said that T. used the hormone Testosterone to change his gender expression from female to male, which changed the hormone levels in his body, especially estrogen and progesterone, causing changes in the uterine lining, leading to abnormal uterine bleeding.
The doctor performed abdominal surgery, completely removed the uterus and both fallopian tubes, kept both ovaries, and sewed to stop the bleeding for T. After the surgery, the patient no longer had a uterus, and the problem of uterine bleeding completely stopped. Male hormone supplementation was also safer than before.
Specialist Doctor II Nguyen Ba My Nhi, Director of the Obstetrics and Gynecology Center, Tam Anh General Hospital, Ho Chi Minh City, said that in Vietnam, the Ministry of Health has not yet licensed the use of hormones for transgender treatment, so Testosterone hormone therapy is only allowed to treat hypogonadism, delayed puberty in men, etc. Most women who desire to change their gender often go abroad for treatment, or buy additional injections themselves.
Testosterone is a powerful hormone that affects the whole body. Taking Testosterone supplements without consulting a doctor, without testing or monitoring is extremely dangerous, and can lead to incorrect dosage, overdose, and dangerous complications such as menstrual disorders and abnormal bleeding, increased risk of cardiovascular disease, liver damage, abnormal increase in red blood cells leading to blood thickening, stroke, and pulmonary embolism. In addition, it affects mental health, behavior (irritability, depression, aggression), hair loss, baldness, and risk of infertility.
A 2024 study published in Pubmed looked at 279 transgender people who started using testosterone, and found that 37% experienced uterine bleeding. The average time to bleeding was 22 months after starting testosterone. These results suggest that a significant proportion of transgender and gender-diverse people using testosterone will experience at least one episode of uterine bleeding.
Doctor My Nhi added that in cases of Testosterone hormone supplementation, periodic check-ups are required every 3-6 months, blood tests are performed, liver and kidney function, blood lipids are assessed, and ultrasound of the ovaries and uterus is performed to detect early abnormalities such as tumors, inflammation, and abnormal bleeding.
In addition, many people are living with the desired gender, but if they do not have the conditions for surgery, the body still has ovaries, uterus, vagina, these parts still need to be checked and protected.
