The Department of Gynecology and Pediatrics, Nghe An Obstetrics and Pediatrics Hospital has successfully operated on 2 cases of Herlyn - Werner - Wunderlich (HWW) syndrome, a rare congenital malformation of the female reproductive and urinary systems.

The first case is NTA (10 years old, residing in Ha Tinh ). The patient was admitted to the hospital with a persistent dull pain in the lower abdomen, without symptoms of nausea or digestive disorders. The patient had not yet reached puberty and had no secondary sexual symptoms. Through a rectal examination, doctors discovered abnormalities in the vaginal area.

Ultrasound and magnetic resonance imaging (MRI) results later determined that A. had a double uterus, a septum in the vagina causing fluid retention, and a solitary left kidney. These are typical signs of HWW syndrome.

The second case is NTHV (11 years old, residing in Thanh Chuong district, Nghe An). The patient had her period and was hospitalized with symptoms of lower abdominal pain and mild constipation. Through diagnosis, doctors also determined that V. had a double uterus, a septum in the vagina and only one kidney on the left side.

After multidisciplinary consultation, both patients were indicated for vaginal surgery. The goal of the surgery was to remove the septum, drain the stagnant fluid, and recreate the passage.

During the surgery, doctors discovered that baby A. had a thick yellow vaginal discharge. For baby V., the discharge showed signs of menstrual blood stagnating for a long time due to the septum.

Both surgeries went smoothly in about 30 minutes, without significant blood loss. After surgery, the two children were closely monitored, given prophylactic antibiotics, and discharged after 5 days in stable condition. Post-operative check-ups showed that the children's health had improved, their abdominal pain had gone away, and ultrasound showed no abnormalities.

According to doctors, HWW syndrome is usually only detected during puberty or at the beginning of menstruation, when symptoms of abdominal pain or urinary disorders appear clearly. About 75% of cases of this syndrome are related to genetic factors, however, the specific cause has not been clearly identified.

Doctors recommend that parents pay special attention if their daughters have persistent lower abdominal pain, unexplained urinary disorders, or abnormalities in their first menstrual cycle. Early examination, timely diagnosis, and proper treatment will help children prevent complications and maintain their fertility in the future.

As in the case of baby A., detection and intervention before puberty helped effectively prevent many dangerous complications such as uterine hematoma, retrograde infection, secondary endometriosis and preserve future fertility.

Herlyn Werner Wunderlich (HWW) syndrome is a rare congenital anomaly of the female reproductive and urinary systems.

The incidence of the disease is unclear, estimated at 0.1-3.8% in the female population, with some reports noting about 1/28,000 cases. The disease is usually detected in puberty or adulthood, at the onset of menstruation.
