Percutaneous transcutaneous aspiration of calcified masses for the treatment of tendinitis

VnExpressVnExpress29/12/2023

Hanoi Under the guidance of ultrasound, the doctor inserts a needle into the site of calcific tendonitis, sucking out the calcified mass to help the patient relieve joint pain.

Calcific tendinitis is a disease caused by the deposition of calcium hydroxyapatite crystals at the attachment points of tendons and ligaments. The disease consists of 4 stages: pre-calcification, calcification, calcification degeneration and post-calcification. Stages 3 and 4 are the time when the patient is most painful and has the most limited mobility, because at this time the body produces enzymes to break down the calcification.

Calcific tendonitis is usually painless, however, when the body releases enzymes to dissolve the calcium, it causes a very painful reaction.

On December 29, Dr. Nguyen Duy Trinh, Deputy Director of the Center for Diagnostic Imaging and Interventional Radiology, Tam Anh General Hospital, Hanoi, said that the optimal treatment for calcific tendonitis is to remove the calcification. Percutaneous calcification aspiration under ultrasound guidance is a minimally invasive method, without bleeding or leaving scars. The method brings high treatment efficiency because it removes almost all the calcification deposited in the tendon, quickly relieving the patient's pain at a reasonable cost.

Like Ms. Ha, 50 years old, had pain in her right hip joint that spread down her thigh. Dr. Nguyen Thi Kim Loan, Department of Musculoskeletal, said that the patient had many diseases such as knee osteoarthritis with inflammatory reaction, bilateral sacroiliitis, femoral neck synovitis, and right iliopsoas tendonitis. In which, calcified tendonitis that developed to stage three was the main cause of pain and needed priority treatment.

Ultrasound results showed a calcified nodule of about 18 mm in the right iliopsoas muscle area, surrounded by fluid and infiltration. The patient was prescribed medication but it did not help, the pain was so severe that he could not walk.

The doctor prescribed ultrasound-guided percutaneous calcification aspiration for the patient, local anesthesia, then inserted a needle into the calcification site under ultrasound guidance, washed and removed all calcification.

The procedure took about 30 minutes, the patient was discharged the same day, the pain reduced by about 20%. By the fourth day, the disease was almost completely cured, no more pain, Ms. Ha could walk lightly, and continued to treat other diseases.

Dr. Loan said that calcific tendonitis is a treatable disease. However, if detected late or treated improperly, the disease can lead to complications such as tendon rupture, local bleeding, infection, causing severe pain and reduced mobility, affecting quality of life. The doctor recommends that patients should see a doctor early when experiencing acute pain, increasing pain that does not go away, or other abnormalities for timely treatment.
