The patient's family said that the night before being admitted to the hospital, the patient drank alcohol during dinner. The next morning, he felt tired, dizzy, and lightheaded, so his family took him to the District Medical Center.
Here, the patient experienced agitation, high blood pressure, rapid deep breathing with signs of respiratory failure. The patient was given initial emergency treatment and then quickly transferred to Phu Tho Provincial General Hospital.
At the hospital, the patient was in a coma, had respiratory failure, had to be ventilated through an endotracheal tube, had cold skin, and purple veins all over his body. Test results showed that the patient had very severe metabolic acidosis, acute kidney failure, severe electrolyte disturbances... suspected to be due to acute poisoning.
Immediately, the doctors performed emergency resuscitation and conducted a blood test for methanol, the result = 48.7 mg/dl. The patient was diagnosed with methanol poisoning with complications of multiple organ failure and severe metabolic acidosis.
The patient continued to receive emergency hemodialysis combined with 20% ethanol administered through the stomach. Continuous hemodialysis was then maintained to correct acid-base disorders and multiple organ failure. The retest result after hemodialysis showed methanol index = 3.2 mg/dl.
After 3 days of intensive treatment, the patient regained consciousness, no longer had metabolic acidosis, no longer had kidney failure, and the endotracheal tube was removed.
According to the assessment of the doctors of the Department of Intensive Care and Anti-Poisoning, this is a very serious case of methanol poisoning due to drinking fake wine containing industrial alcohol, but thanks to early diagnosis, emergency care and timely and active treatment with advanced and modern treatment methods, the patient overcame the critical condition, leaving no sequelae on the eyes, central nervous system and other organs.

Methanol poisoning occurs when the patient mistakenly drinks methanol or drinks alcohol containing Methanol.
Methanol is very toxic because its metabolic products form Formol (Formaldehyde) and Formic acid, which inhibit the central nervous system, damage the optic nerve, cause severe metabolic acidosis and multiple organ failure... Drinking just 5-15ml of Methanol can cause severe poisoning, 15ml or more can cause blindness and 30ml can cause death.
Mild methanol poisoning symptoms include feeling drunk, dizziness, nausea, vomiting, headache. Severe cases will show signs of digestive disorders, vomiting blood, neurological disorders (convulsions, coma, generalized stiffness); respiratory disorders (rapid breathing, acute pulmonary edema); circulatory disorders (rapid pulse, low blood pressure); dilated pupils, retinal hemorrhage and death.
Through the above patient case, Dr. Mai advised the people: “Fake antiseptic alcohol, fake wine, and fake beer mainly contain Methanol, containing very little or no ethanol. Therefore, people need to be more vigilant when using antiseptic alcohol, wine, beer, etc. to avoid using fake or counterfeit goods. Only use alcohol with clear origin, use in a controlled manner.
If you accidentally drink alcohol containing Methanol, especially when symptoms such as low blood pressure, blurred vision, weak limbs, sweating, fever, convulsions, coma appear, the patient needs to be taken immediately to a medical facility for timely treatment, to avoid danger to life and minimize sequelae.
