In the first two episodes of The Edge of Evil, the complicated relationship between Jung Se-ok (Park Eun-bin) and Choi Deok-hee (Sol Kyung-gu) is revealed through the past and present. Deok-hee returned to Korea from Boston to work as a professor at a university hospital and met Se-ok, a genius but uncontrollable student. Both began medical school at the age of 17 and shared an obsession with their work. Se-ok, with her outstanding talent in the operating room, developed arrogance and "god syndrome." When Deok-hee recognized her test results as "perfect," she yearned to be taught by him, but her desperation turned to obsession.

A major conflict broke out when Se-ok lost her chance to go to Boston due to favoritism, which made her angry and Deok-hee slapped her after causing a disturbance. He forbade her from performing surgery, and a year later, when she performed an emergency surgery in Deok-hee's absence, he slapped her again and vowed to revoke her license. The climax came when Se-ok nearly strangled Deok-hee with a tube in front of many witnesses, leading to her kneeling in the rain and screaming in pain when he abandoned her. In the present, Deok-hee discovered Se-ok performing illegal surgery through a video the police showed her, while he hid his brain tumor.

Deok-hee approaches Se-ok, asking her to perform surgery on him so that it won't affect her ability to work, but she refuses, still resentful of him. He threatens to report her if she doesn't agree, but Se-ok retaliates by destroying his medicine when he has an attack. Meanwhile, Se-ok leads a double life: working as a pharmacist by day, performing illegal surgery and even murder by night. She coldly eliminates those who get in her way, with the help of Young-joo (Yoon Chan-young), who manages and covers up her crimes. At the end of episode 2, Deok-hee catches Se-ok burying a victim's body, creating a tense situation that has no end in sight.

Episode 3 begins with Deok-hee (Sol Kyung-gu), a brilliant but cold-hearted neurosurgeon, continuing to observe the murders committed by Se-ok (Park Eun-bin). With his experience and acumen, he understands her outstanding talent as well as her unstable nature. As the police step up their investigation, discovering forensic evidence such as dog hair and medical supplies that link Se-ok to the crime scene, Deok-hee takes action quietly.

He uses his influence to destroy Se-ok's underground network, forcing her pharmacy and anesthesia clinic to close down. The goal is not only to stop her, but also to completely isolate her, pushing her to the brink of death.

Se-ok, deprived of shelter and livelihood, realizes Deok-hee's plot. The pain from her past broken career resurfaces, causing her rage to explode even more violently.

In desperation, she follows Deok-hee to a prestigious awards ceremony where he is honored – a stark contrast to her life of crime. A flashback interrupts: An encounter with a parent who thanks her for saving their child with a successful surgery, giving her a glimmer of hope for the meaning of life. However, this only spurs her to confront Deok-hee head-on, determined not to let him manipulate her further.

The climax comes when Deok-hee, with his talent for psychological manipulation, makes a tempting offer: A research position at a top institution, a chance to regain his lost reputation. He sees this as a "lifeline" for Se-ok, but in reality it is a way to bring her back under control, especially when the police are tightening their siege and his health (brain tumor) makes him need her skills more than ever. Se-ok is faced with a choice: Accept the offer to escape the risk of imprisonment or risk defying Deok-hee, even if it may lead to self-destruction.

The episode ends with Se-ok facing Deok-hee, his eyes burning between a thirst for revenge and an inner conflict, while police sirens sound in the distance, signaling an unpredictable outcome.
