This announcement by the Ministry of Health is stipulated in Decision No. 31/QD-BYT on the publication of data and guidance on exploiting and using data from the integrated electronic health record on VNeID to replace paper records in handling administrative procedures.
Accordingly, the data in the electronic health record on VNeID is interconnected, integrated, and displays complete information with legal validity equivalent to the paper version in administrative procedure documents.
Medical facilities and administrative agencies will not require individuals or organizations to provide paper copies if the corresponding information is already available on the VNeID application.
The Ministry of Health also requires that the exploitation, use, and sharing of data must fully comply with regulations on information security, safety, and personal privacy as stipulated in relevant legal documents, and must also comply with regulations and technical guidelines on connecting, managing, and sharing digital data between agencies.
The data shared in the Electronic Health Book on the VNeID application includes the following information groups: personal administration; allergy history, illnesses, and vaccinations; information on each medical examination and treatment; and a summary of medical records.
Each medical examination and treatment record includes the receiving facility, examination time, hospitalization, and discharge; the type and reason for the examination; the condition and treatment results; the discharge diagnosis; clinical and paraclinical indicators; and medications used in conjunction with the surgery or procedure performed.
A summary of the medical record includes: medical history, clinical course, relevant paraclinical results, treatment methods and approach, prescriptions, instructions, follow-up appointments, and information about the treating physician.
According to regulations, patients or their legal representatives have the right to access and download detailed records of each medical examination and treatment session in PDF format on the VNeID application, ensuring their rights under the Law on Personal Data Protection.

Ministries, departments, and healthcare facilities need to implement data linkage between electronic health records and VNeID.
The Ministry of Health also requested ministries, sectors, localities, and medical examination and treatment facilities to review administrative procedures, adjust information systems, and implement data linkage of electronic health records on the VNeID application.
Mr. Ha Anh Duc, Director of the Department of Medical Examination and Treatment Management, Ministry of Health, said that this is a step forward in administrative reform and data interoperability, allowing the publication, exploitation, and use of integrated electronic health records on VNeID to replace equivalent medical documents in administrative procedures, once the data is fully interconnected, integrated, and displayed.
Previously, the Ministry of Health issued Decision No. 4048/QD-BYT dated December 31, 2025, on the promulgation of the list and implementation plan of the Ministry of Health's shared digital platforms, which includes completing the implementation plan for the integrated electronic health record on the VneID application.
Currently, units are focusing on promoting the implementation of electronic medical records at medical examination and treatment facilities nationwide; the publication of data and guidance on connecting, exploiting, and using the integrated electronic health record data on VNeID to replace paper records in handling related administrative procedures of agencies and units.
