Ho Chi Minh City Three days after having sex, the 24-year-old girl bled and had a fever. The doctor discovered a torn underwear and a severe infection leading to shock and multiple organ failure.

On June 16, Dr. Hoang Le Minh Hien, Hung Vuong Hospital, said that during the emergency surgery, the team discovered that the patient's abdomen contained about 300 ml of blood, pseudomembranes (opaque white inflammatory membranes) throughout the abdomen, and edematous ovaries on both sides. The laceration was about 2 cm long and was oozing a little blood.

The doctor stitched the bleeding and repaired the tear. The patient then went into septic shock, multiple organ failure, required a ventilator, and received intensive treatment. After more than a week, the patient was taken off the ventilator, and respiratory function and signs of infection gradually improved. However, the patient had multiple organ failure after septic shock, so he was transferred to Cho Ray Hospital for continued treatment.

According to Dr. Hien, post-coital bleeding is defined as bleeding during or after intercourse. The estimated rate of post-coital bleeding is about 0.7-9% of cases and is not related to the menstrual cycle. This condition can occur at any age, up to 60% in menopause. In particular, vaginal injury or tearing of the cervix is ​​the most common gynecological emergency of post-coital bleeding.

The foreskin is an anatomical structure in the female adnexa, where the vagina and cervix connect. This area marks the separation between the vagina and the uterus, and is also a thin layer between the adnexa and the abdominal cavity. This is the weakest point of the female internal reproductive organs, playing a role in protecting the sterile environment of the vagina and uterine cavity, preventing dirty microorganisms from entering the intestines.

The main mechanism causing tearing of the vagina is a strong, sudden, and rapid impact that does not allow the vagina to stretch or is too dry, and the mucosa cannot move against the frictional force when in contact.

There are three levels of damage to the vaginal tear. The mildest is a small vaginal scratch, with mild symptoms and the wound can stop bleeding without intervention. In more severe cases, the wound is deep and long but does not go through the entire vaginal wall and requires sutures. The most serious is a large tear of the vaginal tear that communicates with the abdominal cavity and the outside environment, causing infection and risk of damage to organs such as the bladder and intestines.

Two symptoms of damage to the fornix are lower abdominal pain and vaginal bleeding. Prolonged conditions can lead to complications such as sepsis, hemorrhagic shock, septic shock, multiple organ failure, and possibly death. Therefore, doctors recommend that people who experience severe pain during sex, bright red bleeding, blood clots, or prolonged bleeding should stop having sex and go to the hospital immediately for timely treatment.
