That is the case of Ms. D.TP (38 years old) who was hospitalized due to severe pelvic pain below the navel accompanied by bloody stools after using a coffee enema at a private clinic. Previously, the patient had used this method twice, each time one week apart. On the third time, during the enema, the patient felt severe pain in the lower abdomen accompanied by bleeding through the anus. Through questioning, examination and necessary tests, MRI showed an image of retroperitoneal fluid collection, suspected of rectal rupture. Therefore, the doctors decided to perform emergency surgery to treat the injury.
During the surgery, a rupture of the lower third of the rectum caused a retroperitoneal abscess. After 14 days, the patient was discharged and was able to eat and live normally. However, Ms. P will have to undergo another surgery in a few months to close the colostomy.

“In fact, injecting coffee into the anus will reduce the stimulation of the rectum, in the long run, it will cause the rectal reflex to be lost, and if you do not use coffee enema, you will not be able to defecate. At the same time, the loss of rectal reflex also increases the risk of rectal rupture during subsequent enema, because the patient no longer feels the urge to defecate. Therefore, it is very easy to inject too much medicine into the rectum and cause rupture,” said Dr. Nguyen Thanh Khiem.
Studies around the world have shown that there is no convincing evidence that detoxification or cleansing actually removes toxins from the body or improves health. In addition, colon cleansing procedures or enemas - a popular detoxification method, "may have side effects, some of which can be serious". The US National Cancer Institute has reported three deaths that may be related to coffee enemas.
