Special allowance scheme in the healthcare sector to be applied from July 15, 2026.

(Chinhphu.vn) - The Government has just issued Decree No. 192/2026/ND-CP stipulating a number of special allowances in the health sector; monthly support for village and neighborhood health workers and village midwives.

Workers on duty 24/7 during working days are entitled to an on-call allowance ranging from 70,000 to 325,000 VND per person per shift.

This decree stipulates several specific allowances in the healthcare sector, including: on-call allowance; surgical and procedural allowance; epidemic control allowance; and monthly support for village and neighborhood health workers and village midwives.

Applicable subjects

The applicable subjects include:

1. Officials and employees (those working under labor contracts with salaries classified according to the salary scale prescribed by competent agencies of the Party and the State) working at: public medical facilities; commune, ward, and special zone health stations (hereinafter collectively referred to as commune-level health stations), military-civilian health stations; public outpatient emergency medical facilities; facilities for nurturing and caring for meritorious individuals; public social assistance facilities; public forensic and forensic psychiatry organizations including: the National Institute of Forensic Medicine, the Central Institute of Forensic Psychiatry, the Central Institute of Forensic Psychiatry Bien Hoa, regional forensic psychiatry centers, provincial-level forensic centers and the National Coordination Center for Organ Transplantation.

2. Officers, professional soldiers, non-commissioned officers, soldiers, civil servants, public employees, defense workers, and those working under labor contracts in medical facilities of the People's Army; public forensic examination organizations (Military Forensic Institute under the Ministry of National Defense ).

3. Officers, non-commissioned officers receiving salaries, police workers and contract laborers performing specialized medical duties in the People's Public Security, and public forensic examination organizations (including: the Forensic Examination Center under the Institute of Criminal Science, Ministry of Public Security and forensic departments under the Criminal Technical Department, provincial-level Public Security).

4. Members of the National Civil Defense Steering Committee and Civil Defense Commands at all levels, members of the working groups assisting the National Civil Defense Steering Committee and Civil Defense Commands at all levels, and those participating in epidemic control efforts.

5. Healthcare workers providing primary healthcare services in villages, hamlets, and residential areas (collectively referred to as village/hamlet healthcare workers). Healthcare workers providing maternal and child healthcare services in villages, hamlets, and residential areas in ethnic minority and mountainous regions, particularly disadvantaged communes, and coastal and island communes (collectively referred to as village/hamlet midwives).

6. Relevant agencies, organizations, and individuals.

The subjects specified in points 1, 2, 3, and 4 above are collectively referred to as employees.

On-call allowance scheme

Decree No. 192/2026/ND-CP stipulates that employees working 24/7 on weekdays are entitled to the following allowances:

The rate is 325,000 VND/person/shift for: special-grade and grade I medical facilities; the Central Institute of Forensic Psychiatry, and the Central Institute of Forensic Psychiatry Bien Hoa.

The rate is 255,000 VND/person/shift for: Grade II medical facilities, regional forensic psychiatric centers, the Military Forensic Institute under the Ministry of National Defense, the Forensic Examination Center under the Institute of Criminal Science, Ministry of Public Security, and the National Coordination Center for Organ Transplantation (for coordinating the collection, transportation, and preservation of human tissues and organs).

The rate is 185,000 VND/person/shift for: other medical facilities, forensic departments under the Criminal Technical Department of the Provincial Police, public outpatient emergency medical facilities (for on-call duty at emergency points), commune-level health stations, health station branches under commune-level health stations, military-civilian health stations, military-civilian infirmaries, infirmaries of the People's Police, and public social assistance facilities under the Ministry of Health.

The rate is 70,000 VND/person/shift for: facilities caring for and nursing war veterans; and other public social assistance facilities.

If on duty 24/7 in a department or special area, the on-call allowance is calculated at 1.5 times the rate stipulated above.

If on duty 24/7 on a weekly rest day, the on-call allowance is calculated at 1.3 times the prescribed rate; if on duty on public holidays or Tet (Lunar New Year), the on-call allowance is calculated at 1.8 times the prescribed rate.

Employees working 12/24 hour shifts in departments and special areas on weekly rest days, holidays, and Tet (Lunar New Year) as stipulated in point a, clause 2, Article 3 of this Decree are entitled to an allowance equal to 0.5 times the 24/24 hour on-call allowance.

Workers employed in 12/24 hour shifts as stipulated in point b, clause 1, Article 3 of this Decree are entitled to an allowance equal to 0.5 times the 24/24 hour on-call allowance.

Workers who work 16/24 hour shifts as stipulated in point b, clause 1, Article 3 of this Decree are entitled to an allowance equal to 0.75 times the 24/24 hour on-call allowance.

The decree also stipulates that employees on duty under the resident system are entitled to an allowance ranging from 35,000 to 160,000 VND per person per shift, depending on the category of the medical facility.

In addition, employees working 24/7 shifts receive a meal allowance of 40,000 VND per person per shift.

Surgical and procedural allowance scheme

The surgical allowance rates are stipulated as follows:

The lead surgeon, anesthesiologist, or acupuncturist receives a surgical allowance ranging from 100,000 to 560,000 VND per person per surgery, depending on the type of surgery.

Surgical assistants, anesthesia assistants, or acupuncturists are entitled to a surgical allowance ranging from 60,000 to 400,000 VND per person per surgery.

Surgical assistants receive a surgical allowance ranging from 40,000 to 240,000 VND per person per surgery.

The procedure allowance is 0.3 times the allowance for similar surgeries as stipulated above.

Pandemic relief allowance scheme

The decree stipulates that the following individuals are eligible for the epidemic prevention allowance:

Group 1: Individuals directly involved in monitoring and tracking health, investigating and verifying disease outbreaks, collecting and processing samples, conducting tests, and managing cases and outbreaks in the community, medical isolation areas, and healthcare facilities; directly participating in the examination, diagnosis, treatment, and care of people with infectious diseases at healthcare facilities; cleaning, disinfecting, and eliminating disease vectors in outbreak areas, in the community, medical isolation areas, healthcare facilities, residential areas, checkpoints, trails, open routes, border gates, ports, and border regions.

Group 2: Individuals directly involved in community-based vaccination programs and healthcare facilities; those involved in transporting specimens, patients, and quarantined individuals; personnel ensuring the availability of equipment and chemicals; individuals assigned, deployed, or mobilized by the head of the unit (the unit tasked with epidemic prevention and control) to directly participate in epidemic prevention and control activities in the community, medical quarantine areas, healthcare facilities, residential areas, checkpoints, trails, open routes, border gates, ports, and other related epidemic prevention and control activities; members of the National Civil Defense Steering Committee and Civil Defense Commands at all levels participating in epidemic control; members of the support team for the National Civil Defense Steering Committee and Civil Defense Commands at all levels participating in epidemic control; individuals directly involved in inspection, supervision, and support of epidemic prevention and control in localities.

Levels of allowances for epidemic prevention:

Infectious diseases in Group A: those eligible for epidemic control allowance are divided into two groups, Group 1: 420,000 VND/day/person; Group 2: 280,000 VND/day/person.

Infectious diseases in group B: 280,000 VND/day/person.

Infectious diseases in group C: 210,000 VND/day/person.

In addition, the Decree also specifies the allowance for 24/7 epidemic control duty . The allowance for 24/7 epidemic control duty on weekdays is 280,000 VND/day/person, applicable when there is information about an epidemic; on weekends it is 1.3 times the allowance on weekdays; and on holidays and Tet (Lunar New Year) it is 1.8 times the allowance on weekdays.

In addition, workers participating in round-the-clock epidemic control efforts receive a meal allowance of 40,000 VND per person per shift; those directly involved in monitoring, investigating, collecting, processing samples, testing, and verifying the disease, and participating in epidemic control efforts, receive a meal allowance of 40,000 VND per person per day.

Collaborators and volunteers mobilized to participate in combating Group A epidemics during outbreaks also receive a daily allowance of 170,000 - 280,000 VND per person.

Monthly support for village and neighborhood health workers and village midwives.

In addition, Decree No. 192/2026/ND-CP also stipulates a monthly support allowance for village and neighborhood health workers and village midwives. Accordingly, during their term of service, village and neighborhood health workers and village midwives will receive a monthly allowance.

The monthly allowance is 0.7 times or 0.5 times the basic salary, as stipulated below:

A rate of 0.7 times applies to village and neighborhood health workers and village midwives working in: villages with 350 or more households; villages in disadvantaged areas as defined by the Government; and neighborhoods with 500 or more households.

A rate of 0.5 times applies to village and neighborhood health workers and village midwives working in the remaining villages and neighborhoods.

The provincial People's Committee, based on the characteristics, socio-economic development situation, actual needs, and budgetary capacity of the locality, shall submit to the People's Council at the same level a proposal to specifically determine the number of village/neighborhood health workers and village/hamlet midwives; and shall specify the monthly support level for village/neighborhood health workers and village/hamlet midwives, but not lower than the support level mentioned above.

The decree will take effect from July 15, 2026.
