Misunderstandings about adenoiditis in adults

Mr. Q., 35 years old, had recurrent rhinopharyngitis. After going to the doctor, he was diagnosed with residual adenoids. This disease often occurs in children, because in adults, adenoids often disappear and rarely cause many annoying symptoms.

Misunderstandings about adenoiditis in adults

For many years, Mr. D.HQ (Binh Thanh District, Ho Chi Minh City) often suffered from nose and throat diseases, sore throat, recurrent rhinopharyngitis, bought medicine to take but did not get better so he went to the hospital for examination. The results of the ENT endoscopy showed that Mr. Q. had residual adenoids and was in the process of inflammation and congestion.

Master Doctor CKI Pham Thai Duy, ENT Center, Tam Anh General Hospital, Ho Chi Minh City explains that adenoids are lymphoid tissue in the nasopharynx, located at the back of the nose. When breathing, air enters the nose, passes through the nasopharynx - adenoids down the throat, mouth, larynx and then into the lungs.

In young children, the immune system is still developing and is susceptible to viruses and bacteria, so they often have rhinopharyngitis and adenoiditis.

In children, adenoid tissue, in addition to lymphoid tissue responsible for immunity, also contains more adipose tissue and loose connective tissue than in adults.

After the adenoids reach their maximum size at the age of 5-7, they begin to gradually decrease in size. By puberty (around 12-16 years old), in most cases, the adenoids begin to shrink, and the fat in this area disappears (the cause is still unknown).

In adults, adenoids usually exist as a smooth, pale pink tissue in the palate.

However, there are still cases where the adenoid tissue does not shrink but still exists in large size, so it is called residual adenoid. Residual adenoid can still be infected by viruses or bacteria, leading to prolonged rhinopharyngitis. According to medical literature, the rate of residual adenoid in the adult population is about 2.5%.

According to Dr. Duy, if in children, ENT endoscopy detects adenoid hypertrophy, then treatment is medical or adenoidectomy. In adults, when examining and endoscopy, it is necessary to distinguish residual adenoiditis or nasopharyngeal tumor (nasopharyngeal cancer); in some cases, pathological assessment is required to avoid confusion between benign and malignant tumors.

Determining whether a mass in the nasopharynx is a tumor or a residual adenoid is sometimes challenging, because nasopharyngeal tumors in the early stages may not have any symptoms and the initial endoscopic image is also "quite" benign," said Dr. Duy.

Adenoidectomy was indicated because Mr. Q. had been treated with medication many times during the year but the disease still recurred. Surgery helped Mr. Q. significantly reduce the frequency of recurrent rhinitis.

Doctor Thai Duy performed endoscopic adenoidectomy for Mr. Q. using the Coblator method, plasma technology with low-temperature energy waves to completely remove adenoids. Mr. Q. ate and drank normally 3-4 hours after surgery, had good healing ability, and was discharged after 24 hours. After 2 follow-up visits after surgery, his stuffy nose and nasopharyngitis improved.

If not treated promptly, large residual adenoiditis can cause complications such as otitis media, sinusitis, nasal congestion, and runny nose, reducing the patient's quality of life. Therefore, if there are symptoms of recurrent nasopharyngitis, the patient should see a specialist for examination and ENT endoscopy, thereby having timely treatment methods.

Fear of alcohol poisoning

Recently, the Poison Control Center, Bach Mai Hospital provided emergency treatment for 4 cases in Thuong Tin district ( Hanoi ) of methanol poisoning due to drinking the same type of infused wine.

Previously, hospitals also received many cases of poisoning due to drinking infused wine - often advertised as "great tonic" medicinal wine.

According to the patients and their families, these 4 patients attended a family wedding party in Tien Phong commune, Thuong Tin district, where the menu included a type of wine infused with wild apples. In the afternoon of the same day, these 4 patients and another person named C. in Thanh Xuan district (Hanoi) continued to drink the remaining wine, each person drank about 500 - 1,000ml.

After that, the patients showed symptoms of fatigue, dizziness, headache... and were taken to the hospital by their families for treatment. Mr. C. went home to sleep, only 2-3 days later, he had symptoms of stomach pain, vomited blood and died.

Dr. Nguyen Trung Nguyen, Director of the Poison Control Center, Bach Mai Hospital, said: 4 patients in Thuong Tin district were hospitalized with headaches, blurred vision, severe metabolic acidosis, and very high blood methanol levels.

In addition to eye damage, patients are also at risk of brain damage. Test results of alcohol samples the patients drank at the wedding in Thuong Tin showed that the concentration of industrial alcohol methanol was 34%.

According to Dr. Nguyen Trung Nguyen, industrial alcohol methanol when entering the body will cause damage, especially damage to the optic nerve and brain.

However, because methanol metabolizes slowly and discreetly, there are cases where the eyes and brain have been damaged but no symptoms have been shown and so the patient has not gone to the doctor.

Through many cases of industrial methanol poisoning, in addition to deaths and severe poisoning, doctors also encountered cases with normal symptoms but when actively tested, the concentration of industrial methanol in the blood was very high and had to filter the blood and detoxify. If not treated promptly, these patients may suffer partial damage to the eyes and brain.

For a long time, soaked wines have often been advertised as medicinal wines, soaked with medicinal herbs and roots that have the effect of nourishing and treating many different diseases.

From the medicinal wine recipes of Thap toan dai bo, Minh Mang thang, ginseng, sea horse... that have been "passed down by word of mouth", many people also make and soak wolfberry wine, ginseng wine, longan wine... to wine of hawthorn, water chestnut, banana seeds... to nourish the body.

Many people even believe that herbs and roots are natural, benign foods, so drinking wine soaked with any type of plant, root, or leaf that has medicinal and nutritious effects can achieve "great tonic" effects.

Dr. Nguyen Trung Nguyen said that the Center regularly receives and treats many patients who are hospitalized due to poisoning after drinking wine soaked with tree roots to treat back pain, knee pain, and to nourish the body.

In addition to cases of methanol poisoning, some cases of testing of alcohol samples brought by patients and blood tests of patients detected salicylates originating from plant roots.

Salicylate poisoning can lead to serious health consequences. Patients experience dizziness, nausea, tinnitus, convulsions, coma, hypotension, brain damage and kidney damage, and are at high risk of death.

According to doctors' recommendations, when using medicinal wine products, wine soaked with fruits, roots, especially poisonous roots, one must be extremely careful and follow the instructions of an experienced physician.

In addition, the dosage or time in the process of soaking the plants is also different. Because when self-processing or soaking with alcohol, if you do not know how to process to remove toxins, poisoning from these soaked wines can also lead to death.

Hanoi: More than 4,000 food and beverage establishments decreased compared to the same period

According to the report of the Hanoi Department of Food Safety and Hygiene, the city currently has 72,671 establishments producing, processing, trading food, catering services, and street food (a decrease of 4,136 establishments compared to the same period in 2023) throughout the city.

Of which, the health sector manages 39,882 facilities (city level: 4,736 facilities; district, town level: 9,227 facilities; commune, ward, town level: 25,919 facilities).

In recent times, Hanoi has maintained food safety assurance for food services and street food in 100% of wards and towns in the city, and 60 civilized streets ensuring food safety in 30 districts, towns and cities.

From the beginning of 2024 to now, the authorities have inspected and supervised 5,820/5,820 street food establishments (accounting for 100%). Of these, 408 establishments were inspected at festivals, fairs, and exhibitions, 1,489 establishments in tourist areas, 1,390 establishments in public places (sidewalks, bus stations, train stations, etc.), and 2,533 street vendors. The rate of establishments meeting the prescribed criteria is 83.7% (4,872 establishments).

The authorities have inspected and supervised 35,146/35,146 food service establishments (100%). The rate of establishments meeting the prescribed criteria is 84.5%.

The city has maintained a model of food safety control at crowded parties to proactively prevent food poisoning in the area in 2024 with 440 communes, wards, and towns/20 districts, towns, and cities.

Since the beginning of the year, the delegations have supervised and consulted on 16,572 banquets, guided the implementation of food safety regulations in families organizing large gatherings and signed a commitment on food safety reaching 100%.

Hanoi also maintains and replicates the food safety control model for primary school canteens in 15 districts, towns and cities with 324 schools.

Since the beginning of 2024, the city has organized a monitoring and inspection of food origin traceability at 15 primary school canteens and 10 food production, processing and trading establishments supplying raw materials to primary school canteens in 15 districts, towns and cities to implement the model "Controlling food safety in school canteens".

District and county levels have inspected and supervised 324 establishments, of which 298 establishments, accounting for 91.9%.

In addition, the city has also piloted a model to control food safety in catering services and street food around school gates in Trang Tien and Hang Trong wards in Hoan Kiem district.

Since the beginning of the year, the authorities have inspected and supervised 65/105 establishments; the rate of meeting the criteria is 89.2% (58/65 establishments); 1 establishment was fined 2 million VND; 3 establishments were warned on the spot; rapid testing reached 103/108 samples (rate 95.4%).
