On April 14, pregnant woman PTQN (22 years old, in Ngoc Son commune, Thach Ha district) showed signs of amniotic fluid leakage, so her family took her to the Provincial General Hospital for monitoring. Here, after examination, the signs of amniotic fluid leakage were under control, so the pregnant woman was asked to continue staying in the hospital for monitoring.

On the afternoon of April 15, the pregnant woman had a stomachache and her water broke. At this time, midwife Tran Thi Xuan (Department of On-Demand Treatment) examined the pregnant woman and discovered that the umbilical cord and the fetus's arm had fallen down to the cervix, under the pregnant woman's vagina.


At this time, the doctors gave the mother oxygen, while midwife Tran Thi Xuan pushed the umbilical cord and the baby's head up to avoid compression of the umbilical cord; at the same time, the mother was transferred to a stretcher with her head low and her legs raised to travel nearly 300m to the operating room.


The pregnant woman was transferred to the emergency operating room according to the hospital's red alert procedure. Obstetricians and anesthesiologists were mobilized and coordinated smoothly to perform the emergency surgery. The entire process of detecting, consulting, and moving the patient to the operating room for emergency cesarean section took just over 5 minutes (the baby girl weighed 3kg).

Doctor Le Huyen Anh - Deputy Head of the Department of Treatment, who directly performed the surgery, said: The case of the mother N. was very lucky because she had been closely monitored by doctors and midwives at the department beforehand, otherwise it would have been very difficult to save the baby and the mother. In the case of monitoring at home, even if it was detected in time, the risk of death for the mother and baby would still be very high.

Umbilical cord prolapse is one of the obstetric complications that cause acute fetal distress. If the emergency cesarean section is delayed, the fetus will suffer from respiratory failure, coma and death, or if it survives, the baby is susceptible to brain damage due to lack of oxygen leading to sequelae. In cases of umbilical cord prolapse outside the vagina, if not detected and treated promptly, the mortality rate for the baby is very high, because the umbilical cord is compressed between the fetus and the pelvic wall, or because when it prolapses outside the vagina, the blood supply of the umbilical cord to the fetus is interrupted due to compression of the umbilical cord blood vessels.

To avoid this complication, pregnant women need to have regular check-ups; if the pregnant woman shows any unusual signs, the family needs to take her to the emergency room at a hospital with a team of experienced doctors to handle the situation quickly to remove the fetus as quickly as possible.
