During pregnancy monitoring in Japan, the pregnant woman was diagnosed with twin-to-twin transfusion syndrome when the fetus was 18 weeks old.

This is a dangerous complication and is very common in cases of twins sharing a placenta. This syndrome occurs when blood from one fetus is transferred to the other fetus through the common blood vessels, seriously affecting the development of both fetuses.

When the mother returned to Vietnam at the 20th week of pregnancy, doctors at the Fetal Intervention Center of Hanoi Obstetrics and Gynecology Hospital examined her and diagnosed her with stage III twin-to-twin transfusion syndrome.

One fetus was severely retarded, while the other showed signs of overload due to receiving too much blood. Furthermore, the mother also had a large uterine fibroid (71x94mm), which further complicated the situation.

Faced with the dangerous situation, Dr. Phan Thi Huyen Thuong, Deputy Director of the Fetal Intervention Center, and her team of doctors decided to perform fetal intervention surgery using laser photocoagulation. This is a modern method that helps prevent uncontrolled blood transfusion between the two fetuses, thereby protecting the health of both mother and child.

After surgery, the condition of the mother and fetuses was stable, they were discharged and continued to monitor their pregnancy at the hospital. Doctor Thuong said that this technique has helped save the lives of thousands of twins suffering from blood transfusion complications each year in Vietnam. With a high success rate and few side effects, laser photocoagulation has become an effective treatment option, protecting the lives of both mother and child in such dangerous situations.

According to doctors at the Fetal Intervention Center, for every 100 cases of twins sharing a placenta, there are 20-30 cases of twin-to-twin transfusion syndrome. This is a serious condition in which one fetus donates blood and becomes anemic, slow to develop, and has low amniotic fluid. Meanwhile, the fetus receiving blood is overloaded, leading to impaired heart and blood vessel function, causing fetal edema.

If not detected and treated promptly, twin-to-twin transfusion syndrome can lead to death for both fetuses in the uterus. Those who are lucky enough to survive will face severe neurological sequelae. Therefore, the advent and application of laser photocoagulation in fetal intervention has played an extremely important role in saving the lives of fetuses suffering from this dangerous complication.

Laser photocoagulation is an advanced technique used to treat twin-to-twin transfusion syndrome. By using a laser to block the blood vessels shared by the twins, doctors can stop the uncontrolled blood transfusion while protecting the normal development of both fetuses. This method not only helps reduce the risk of death but also limits serious neurological sequelae for the fetus.

Timely emergency surgery for patient with abdominal trauma

Doctors at Ha Dong General Hospital have just successfully performed emergency surgery on a patient with abdominal trauma, peritonitis due to multiple ruptures of the small intestine, crushed sigmoid colon, severed sigmoid colon artery, and intra-abdominal bleeding due to a concrete pole falling on him. This is one of the most urgent emergency surgeries that needs to be performed quickly and accurately to save the patient's life in time.

Patient Bui AD (40 years old, residing in Bao Hieu commune, Yen Thuy district, Hoa Binh province) was lying in a hammock when a concrete pole holding the hammock fell on him.

The patient was admitted to Ha Dong General Hospital for emergency treatment with severe abdominal pain, abdominal cramps, and abdominal distension. Upon admission, the doctors conducted a clinical and paraclinical examination. The abdominal CT scan showed free air and fluid effusion, and monitored small intestinal loop damage around the navel.

Through specialist consultation, doctors diagnosed a closed abdominal injury, ruptured small intestine causing generalized peritonitis, with a high risk of death. The patient was quickly assigned emergency surgery.

During the surgery, the surgical team led by Dr. Bui Duc Duy - Head of the Department of Gastroenterology, examined and assessed the patient's abdomen, which contained a lot of digestive fluid and food spreading throughout the abdomen due to multiple ruptures in the 80cm long small intestine, crushing the entire sigmoid colon serosa, and severing the sigmoid colon artery, with about 1000ml of blood in the abdomen.

The surgeon performed a small bowel resection with multiple ruptures at the lateral-lateral anastomosis, resection of the sigmoid colon, and placement of both ends of the bowel to create an artificial anus. The abdominal cavity was cleaned and an abdominal drain was placed. 24 hours after surgery, the patient was conscious and in stable health.

Previously, many cases of abdominal trauma and complex rupture of multiple abdominal organs such as liver, kidney, stomach, duodenum, etc. were successfully saved by surgery by doctors at Ha Dong General Hospital.

Thereby affirming the solid expertise, experience, and effective emergency treatment capacity of the medical team at the hospital, reducing the mortality rate, bringing life to critically ill patients on the verge of death.
