Lost 15 kg after 2 months of gastric bypass surgery

A 65-year-old female patient, grade three obese, unable to lose weight in the usual way, had gastric bypass surgery and lost 15 kg in two months.

According to the female patient, she weighed 97 kg, had a body mass index (BMI) of nearly 42, and was at the highest level of obesity.

She has been overweight for many years, and recently her weight has been uncontrolled. She also has a herniated disc, cervical and lumbar spine degeneration, and insomnia, which makes it difficult for her to walk and affects her daily activities. This is the reason why she cannot choose a suitable sport to lose weight.

According to Asian classification, a person has normal weight (not pregnant) when BMI is between 18.5 and 23.

Dr. Do Minh Hung, Director of the Center for Endoscopy and Endoscopic Surgery of the Digestive System, Tam Anh General Hospital, Ho Chi Minh City, said that the patient was indicated for surgery to reduce and shape the stomach tube.

After anesthetizing the patient, the surgeon laparoscopically reduces the stomach through small incisions in the abdomen. The surgical team removes 80% of the stomach according to the shape determined by the bougie tube (a device that shapes the new volume of the stomach), removes the fundus containing ghrelin (a hormone that stimulates appetite), and sutures the wound.

Dr. Hung explained that the purpose of gastric reduction is to create an energy deficit in the body, reducing the volume of the stomach to help patients after surgery reduce their need for food consumption. Eliminating ghrelin helps patients control their appetite effectively.

One day after surgery, the patient can eat, walk normally, and be discharged from the hospital. After surgery, she must follow the doctor's instructions on diet, combined with exercise to achieve the ideal weight, and improve the accompanying health indicators. One week later, the patient lost 7 kg, after two months lost 15 kg, and her health improved significantly.

Her joint pain has decreased, she can walk and sleep better without the help of sleeping pills. According to the doctor's advice, Ms. Hien's goal is to lose 20 kg in 6 months, about 30 kg in 12 months.

Doctor Minh Hung said that there are many techniques to treat obesity, of which gastric sleeve surgery is the method performed most in the world .

Patients usually lose 75% of their excess weight after one year. This is the last resort, indicated for people with a BMI of 40 or higher, or 35 or higher, and at least one other disease such as high blood pressure, diabetes, sleep apnea, high cholesterol, osteoarthritis, etc. Doctors in some Asian countries choose a BMI of 37 or 32 with metabolic disease to indicate surgery.

Previously, information from Viet Duc Friendship Hospital said that the facility had just received a female patient NTD, 31 years old, 1.53m tall but weighing 100kg. With an "overweight" body, D. always feels inferior, stressed, anxious, often has insomnia, and is easily irritated.

The patient said that since childhood she was chubbier than her peers, and when she reached puberty she could no longer control her weight.

D.'s obesity made her daily life and health difficult, so she tried many weight loss methods such as exercise and dieting, but they were ineffective. With a BMI of 43, at Viet Duc Friendship Hospital, doctors diagnosed her with grade 3 obesity.

According to Dr. Bui Thanh Phuc, Deputy Head of the Department of Gastroenterology Emergency at Viet Duc Friendship Hospital, NTD is one of nearly 300 obese cases who have come to Viet Duc doctors when they cannot control their weight, many cases fall into a state of self-consciousness and depression because of being overweight or obese.

Patient D. came to the hospital when he was already grade 3 obese, had high blood sugar (glucose level 9.68 mmol/L), and had diabetes and menstrual disorders.

With the advice of doctors, patient D. chose the method of creating a vertical tube stomach to reduce food absorption into the body. Obesity surgery is considered a fairly sustainable method.

Obesity is on the rise in Vietnam and Southeast Asian countries. Obesity not only affects the patient's body shape and psychology, but is also a potential cause of many cardiovascular diseases, metabolic disorders, type 2 diabetes, sleep apnea, and bone and joint diseases. The longer obesity lasts, the higher the risk of affecting the spine, causing degeneration of the spine and discs.

Improve weight, the accompanying health indicators also change in a positive direction. Doctor Minh Hung recommends preventing obesity by adjusting lifestyle, maintaining physical activity, balancing nutrient groups, calculating daily energy supply. You should choose a sport that suits your health, goals and personal interests.

Patients needing obesity treatment should choose a reputable medical facility and a highly skilled doctor for a smooth surgery and to prevent complications.

It is known that according to the classification of Asians, a person has normal weight (not pregnant) when BMI is between 18.5 and 23.

If BMI is between 23 and under 25, it is considered overweight, pre-obese; BMI from 25 to 30 is considered grade 1 obesity; from 30-35 is grade 2 obesity and over 35 is grade 3 obesity.

According to the World Health Organization (WHO), there are currently 6.5 million obese people in the world who need treatment, and the cost of obesity treatment is huge. The mortality rate related to obesity is twice the mortality rate of breast and colorectal cancer combined.

In Vietnam, the rate of overweight and obesity tends to increase rapidly, especially in urban areas. According to the results of the 2019-2020 National Nutrition Survey by the National Institute of Nutrition, the rate of overweight and obese children increased 2.2 times, from 8.5% (in 2010) to 19% (in 2020).

According to statistics from the Ministry of Health, in 2020 alone, the rate of overweight and obesity in urban areas reached 26.8%, in rural areas it was 18.3% and in mountainous areas it was 6.9%. Previously, the National Institute of Nutrition also announced that the obesity rate in inner-city children in Ho Chi Minh City exceeded 50%, in Hanoi it exceeded 41%.

To limit the consequences caused by obesity, doctors recommend that because this is a chronic disease, it should be treated early. Obesity not only affects daily life and aesthetics, but also causes many risks such as cardiovascular diseases, diabetes, fatty liver, cirrhosis, musculoskeletal diseases, infertility, etc.

Obesity and overweight also increase the risk of death compared to patients with normal weight. The principle of treating overweight and obesity is to increase energy expenditure and reduce food intake.

The main cause of obesity in Vietnam, according to doctors, is the improvement in the living standards of Vietnamese people. The living standards of Vietnamese people are starting to approach those of developed countries. Energy-rich foods and fast foods are increasingly appearing on Vietnamese dining tables.

Meanwhile, healthy foods such as green vegetables and fruits are lacking and replaced by fried chicken, fast food, soft drinks... these foods are the cause of overweight and obesity.

Doctors also warn that obesity is a modern disease that is increasing at an alarming rate, threatening to reduce life expectancy and affect quality of life, leading to a series of consequences for other diseases such as high blood pressure, myocardial infarction, heart failure, diabetes, infertility, cancer, etc.

Treatment of overweight and obesity is a multidisciplinary treatment: Nutrition, physical activity, weight loss medication, surgery, psychological counseling. Young patients need early treatment to avoid affecting their exercise regimen and reproductive health later on.
