Ms. Ton Thi Ngoc G., 35 years old, from a province in the Southwest, has a congenital defect of complete cleft lip and palate, from the upper lip through the dental arch to the uvula.

Since she was a child, her family took her to many places for surgery to close her cleft lip and palate. However, the consequences of the deformity caused her upper jaw to be severely underdeveloped. The mid-face fell back, causing a severe overbite - also known as an "underbite" or "plow face".

The fact that her jaws cannot touch each other makes it difficult for her to eat, speak, clean her teeth, and maintain her appearance. Expressing or sharing something also becomes a big barrier. She can barely speak clearly, making it difficult for others to hear. Many times, she has to ask her relatives, especially her husband, to say what she wants to share.

Dr. Vu Trung Truc, Deputy Head of the Department of Maxillofacial, Plastic and Aesthetic Surgery - who directly performed the surgery, shared that this was a very difficult surgery. The maxillary bone was excessively hypoplastic, leading to a gap of more than 10mm between the upper and lower incisors, and the soft tissue was heavily scarred due to multiple surgical interventions, making the dissection and bone movement complicated.

To ensure accuracy, the surgical team decided to apply modern 3D pre-operative simulation technology combined with a surgical navigation system. During the surgery, the jaw bones were cut and moved precisely to the new, pre-calculated position.

The 5-hour surgery with interdisciplinary coordination was a great success, making patient G's dream come true.

According to Associate Professor, Dr. Nguyen Hong Ha, Head of the Department of Maxillofacial Surgery, Plastic and Aesthetic Surgery, cleft lip and palate deformities are quite common in developing countries like Vietnam.

Treatment for these cases must be carried out in stages, lasting until the child is no longer growing and requires close coordination between many specialties such as orthodontics, anesthesia, resuscitation, otolaryngology, psychological counseling, etc. If a reasonable treatment plan is not made early, correcting the sequelae later will be extremely difficult.

After jaw surgery, Ms. G. continued to have her nose corrected with autologous rib cartilage and is in the process of completing orthodontics. Her face has changed significantly with adjusted jaw structure, chewing function and pronunciation clearly improved. With a more harmonious appearance, she is confident when going out, no longer having to endure curious and scrutinizing looks like before.

In particular, after rhinoplasty, her airway was completely restored. Before that, she could only breathe through her mouth for many years.
